脐带血IL-6和IL-10基因多态性及其血清水平与早产儿感染的关联

摘　　要：目的探究脐带血白细胞介素(IL)-6基因位点-572C/G多态性、IL-10基因位点-1082A/G多态性及IL-6、IL-10血清水平与早产儿感染的关联。方法选取2015年1月-2020年12月湖北省妇幼保健院114例早产儿感染患儿为感染组,同期60名无感染早产儿为未感染组,采用新生儿危重病例评分表(NCIS)评价病情严重程度,聚合酶链反应限制性片段长度多态性(PCR-RFLP)法检测IL-6-572C/G、IL-10-1082A/G基因多态性,酶联免疫吸附法检测脐带血血清IL-6、IL-10水平。结果感染组NCIS评分低于未感染组(P<0.05);感染组IL-6-572C/G等位基因G频率、GG基因型频率以及IL-10-1082A/G等位基因A频率、AA基因型频率高于未感染组(P<0.05);感染组脐带血血清IL-6、IL-10表达水平均高于未感染组(P<0.05)。感染组中IL-6 GG型脐带血血清IL-6表达高于CG型、CC型,且CG型高于CC型,但IL-10不同基因型IL-10表达水平比较,无统计学差异。预后不良组脐带血血清IL-6、IL-10表达均高于预后良好组(P<0.05)。结论早产儿感染进一步加重病情,其发生及预后可能与脐带血IL-6-572C/G、IL-10-1082A/G基因多态性以及血清IL-6、IL-10水平有关。OBJECTIVE To explore the association between the polymorphism at-572 C/G locus of interleukin-6(IL-6)gene,polymorphism at-1082 A/G locus of interleukin-10(IL-10)gene,levels of IL-6 and IL-10 in umbilical cord blood and the infection in preterm infants.METHODS A total of 114 preterm infants with infection who were treated in Hubei Maternal and Child Health Care Hospital from Jan 2015 to Dec 2020 were assigned as the infection group,meanwhile,60 preterm infants without infection were chosen as the no infection group.The severity of disease was evaluated by Neonatal Critical Illness Score(NCIS),and the gene polymorphisms of IL-6-572 C/G and IL-10-1082 A/G were detected by means of polymerase chain reaction restriction fragment length polymorphism(PCR-RFLP).RESULTS The NCIS of the infection group was significantly lower than that of the no infection group(P<0.05).The frequencies of allele G and GG genotype at IL-6-572 C/G and the frequencies of allele A and AA genotype at IL-10-1082 A/G were significantly higher in the infection group than in the no infection group(P<0.05).The expression levels of umbilical cord blood IL-6 and IL-10 of the infection group were significantly higher than those of the no infection group(P<0.05).In the infection group,the expression level of umbilical cord blood IL-6 of the infants with IL-6 GG genotype was significantly higher than that of the infants with genotype GG and genotype CC,the CG genotype was higher than CC genotype,however,there was no significant difference in the expression level of IL-10 among the infants with different IL-10 genotypes.The expression levels of umbilical cord blood IL-6 and IL-10 of the poor prognosis group were significantly higher than those of the favorable prognosis group(P<0.05).CONCLUSION The infection in the preterm infants may further aggravate the illness condition,the occurrence of infection and the prognosis may be associated with gene polymorphisms of umbilical cord blood IL-6-572 C/G,IL-10-1082 A/G and the levels of serum IL-6 and IL-10.
