摘  要:目的 探讨高危型人乳头瘤病毒(HPV)感染对体外受精/卵胞浆内单精子注射-胚胎移植(IVF/ICSI-ET)患者外周血单个核细胞(PBMCs)Toll样受体4(TLR4)、核转录因子-κB(NF-κB)表达的影响。方法 选择2018年8月-2020年5月海南现代妇女儿童医院收治的拟进行IVF/ICSI-ET的患者为研究对象,根据HPV检查结果将高危型HPV感染患者104例及非高危型HPV感染患者152例,分别纳入高危型组及非高危型组。采集外周静脉血检测PBMCs中TLR4、NF-κB表达水平,常规进行IVF/ICSI治疗,收集IVF/ICSI治疗参数,随访统计妊娠结局。结果 高危型组PBMCs中TLR4、NF-κB表达水平高于非高危型组(P<0.05),两组IVF/ICSI参数及临床妊娠率、活产率、早产率、出生缺陷率比较,均无统计学差异;高危型组流产率高于非高危型组(P<0.05),两组妊娠结局良好者TLR4、NF-κB表达水平均低于妊娠结局不良者(P<0.05)。结论 高危型HPV感染可导致IVF/ICSI-ET患者PBMCs中TLR4、NF-κB表达水平的升高,这一生理病理变化可能是高危型HPV感染患者不良妊娠结局发生的机制。OBJECTIVE To explore the influence of high-risk human papillomavirus(HPV) infection on expression levels of Toll-like receptor 4(TLR4) and nuclear transcription factor-κB(NF-κB) in peripheral blood mononuclear cells(PBMCs) of patients undergoing in vitro fertilization or intracytoplasmic sperm injection and embryo transfer(IVF/ICSI-ET). METHODS The patients who were supposed to underwent IVF/ICSI-ET in Hainan Modern Women’s and Children’s Hospital from Aug 2018 to May 2020 were recruited as the study subjects. According to the results of HPV test, 104 patients who had high-risk HPV infection were assigned as the high-risk group, and 152 patients who had non-high-risk HPV infection were assigned as the non-high-risk group. The peripheral venous blood specimens were collected to detect the expression levels of TLR4 and NF-κB in PBMCs, the routine IVF/ICSI was carried out, the IVF/ICSI-ET parameters were collected, and the pregnancy outcomes were followed up and statistically analyzed. RESULTS The expression levels of TLR4 and NF-κB in PBMCs were significantly higher in the high-risk group than in the non-high-risk group(P<0.05). There were no significant differences in the IVF/ICSI parameters, clinical pregnancy rate, live birthrate, premature delivery rate and birth defect rate between the two groups. The miscarriage rate of the high-risk group was higher than that of the non-high-risk group(P<0.05). Among the two groups of patients, the expression levels of TLR4 and NF-κB of the patients with favorable pregnancy outcomes were significantly lower than those of the patients with poor pregnancy outcomes(P<0.05). CONCLUSION The high-risk HPV infection may lead to the rise of expression levels of TLR4 and NF-κB in PBMCs of the IVF/ICSI-ET patients, and the physiological and pathological changes may be the mechanisms for pathogenesis of poor pregnancy outcomes of the patients with high-risk HPV infection.
