摘  要:目的 探讨儿童肺炎链球菌感染急性中耳炎(AOM)血清钙结合蛋白S100钙结合蛋白A8(S100A8)、S100A9、S100A12表达及其临床意义。方法 选择2018年12月-2020年12月山西省儿童医院、山西医科大学第二医院耳鼻喉科收治肺炎链球菌感染AOM患儿74例纳入AOM组,选择同期医院体检健康儿童60名为对照组,收集两组临床资料;AOM组根据病情严重程度分为轻度组(27例)、中度组(26例)及重度组(21例),根据疗效分为好转组(48例)、非好转组(26例),比较各组血清S100A8、S100A9、S100A12水平。结果 AOM患儿以单耳、咽鼓管和急性化脓性感染为主;74株检出肺炎链球菌对磺胺甲噁唑/甲氧苄啶、红霉素、克林霉素3种抗菌药物耐药率较高(95.95%、83.78%、74.32%),对利奈唑胺和万古霉素耐药率均为0。AOM组血清S100A8、S100A9、S100A12水平高于对照组(P<0.05)。重度组血清S100A8、S100A9、S100A12水平高于轻度组、中度组,中度组高于轻度组(P<0.05);治疗48 h后,两组血清S100A8、S100A9、S100A12水平均较治疗前下降,且好转组低于非好转组(P<0.05)。受试者工作特征曲线(ROC)分析结果显示S100A8、S100A9、S100A12单指标用于AOM诊断曲线下面积(AUC)分别为0.755、0.763、0.782,低于3指标联合时的0.852(P<0.05)。结论 肺炎链球菌感染AOM患儿血清中S100A8、S100A9、S100A12水平可用于AOM诊断、严重程度及疗效评估。OBJECTIVE To explore the expression of serum calbindin S100 calbindin A8(S100 A8), S100 A9 and S100 A12 in children with Streptococcus pneumoniae infection-induced acute otitis media(AOM) and analyze the clinical significance. METHODS A total of 74 children with S.pneumoniae infection-induced AOM who were treated in otolaryngology department of Shanxi Children’s Hospital and the Second Hospital of Shanxi Medical University from Dec 2018 to Dec 2020 were assigned as the AOM group, meanwhile, 60 healthy children who received physical examination were chosen as the control group. The clinical data were collected, the children of the AOM group were divided into the mild group with 27 cases, the moderate group with 26 cases and the severe group with 21 cases according to the severity of AOM and were divided into the improved group with 48 cases and the non-improved group with 26 cases according to the therapeutic effect. The levels of serum S100 A8, S100 A9 and S100 A12 were compared among the groups. RESULTS The children with AOM were characterized by single ear, eustachian tube and acute suppurative infection. The drug resistance rates of 74 S.pneumoniae isolates to sulfamethoxazole-trimethoprim, erythromycin and clindamycin were 95.95%, 83.78% and 74.32%, respectively;the drug resistance rates to linezolid and vancomycin were 0. The levels of serum S100 A8, S100 A9 and S100 A12 of the AOM group were significantly higher than those of the control group(P<0.05). The levels of serum S100 A8, S100 A9 and S100 A12 of the severe group were significantly higher than those of the mild group and the moderate group, and the levels of above indexes of the moderate group were significantly higher than those of the mild group(P<0.05). The levels of S100 A8, S100 A9 and S100 A12 of the two groups were significantly lower after the treatment for 48 hours than before the treatment, and the levels of above indexes of the improved group were significantly lower than those of the non-improved group(P<0.05).
