椎弓根内固定脊柱融合治疗腰椎间盘突出症的疗效分析

摘　　要：目的:观察椎弓根内固定脊柱融合术治疗腰椎间盘突出症的临床治疗效果。方法:收集本院腰椎间盘突出症患者86例,随机分成对照组43例、治疗组43例,分别给予传统按摩治疗和椎弓根内固定脊柱融合术治疗。观察并记录分析视觉疼痛模拟评分(VAS)、计时起走测试评分(TUG)、日本骨科协会评估治疗评分(JOA)、匹兹堡睡眠质量指数(PSQI)、临床疗效和不良反应率。结果:治疗后,VAS、TUG组间比较,治疗组低于对照组(P<0.001);JOA组间比较,治疗组高于对照组(P<0.001);PSQI组间比较,治疗组显著优于对照组(P<0.001);不良反应发生率组间比较,治疗组低于对照组(P<0.05);临床治疗有效率组间比较,治疗组高于对照组(P<0.05)。结论:椎弓根内固定脊柱融合术治疗老年腰椎间盘突出症临床效果确切,可改善患者的睡眠质量,疗效显著。Objective:To observe the clinical effect of pedicle internal fixation and spinal fusion in the treatment of lumbar disc herniation.Methods:86 patients with lumbar disc herniation in our hospital were randomly divided equally into the control group and the treatment group.Visual pain analogue scale(VAS),time up and go(TUG),treatment of grade assessment of the Japanese orthopaedic association(JOA),pittsburgh sleep quality index(PSQI),clinical efficacy and adverse effects rate were observed,and then all the data were recorded and analyzed.Results:After treatment,the VAS and TUG scores of the two groups were significantly lower than those before treatment(P<0.001).The comparison results between VAS and TUG groups showed that the treatment group was less than the control group(P<0.001);the comparison results between JOA groups showed that the treatment group was greater than the control group(P<0.001);and the comparison results between PSQI groups showed that the treatment group was significantly better than the control group(P<0.001).The incidence of adverse reactions in the treatment group was lower than that in the control group(P<0.05).The effective rates of clinical treatment were compared,which shown that the treatment group greater than the control group(P<0.05).Conclusion:The clinical effect of pedicle internal fixation and spinal fusion in the treatment of elderly patients with lumbar disc herniation is obvious.It can improve the clinical symptoms and the sleep quality of patients.Our results have revealed that pedicle internal fixation and spinal fusion have significant clinical value.
