床旁颅脑超声对新生儿颅内出血的诊断价值研究

摘　　要：目的 探讨床旁颅脑超声对新生儿颅内出血的诊断价值。方法 80例经脑血管造影确诊为颅内出血的新生儿作为研究对象,所有患儿出生3 d内均接受颅脑超声检查及CT检查。对比颅脑超声检查及CT检查的诊断符合情况,颅内出血具体分为脑室出血、脑实质出血及硬膜下出血。观察超声对不同胎龄患儿颅内出血检出情况。结果 颅脑超声检查的诊断准确率为95.00%高于颅脑CT检查的80.00%,差异有统计学意义(P<0.05)。经颅脑超声检查,早产儿与足月儿出血部位对比,差异无统计学意义(P>0.05)。足月儿的超声检查漏诊率为30.00%高于早产儿的6.67%,差异有统计学意义(P<0.05)。结论 颅脑超声在新生儿颅内出血诊断中的应用价值较高,可较好判断新生儿颅内出血情况,为临床治疗提供影像学依据。Objective To discuss the diagnostic value of bedside craniocerebral ultrasonography in neonatal intracranial hemorrhage. Methods 80 newborns diagnosed with intracranial hemorrhage by cerebral angiography were studied, and all of them underwent craniocerebral ultrasonography and CT examination within 3 d of birth. The diagnostic conformity of cranial ultrasonography and CT examination was compared,and intracranial hemorrhage were classified as ventricular hemorrhage, parenchymal hemorrhage and subdural hemorrhage. The detection of intracranial hemorrhage by ultrasonography was observed in children of different gestational ages. Results The diagnostic accuracy rate of craniocerebral ultrasonography was 95.00%, which was higher than 80.00% of CT examination, and the difference was statistically significant(P<0.05). Craniocerebral ultrasonography showed no significant difference in bleeding sites between premature infants and full-term infants(P>0.05). The missed diagnosis rate 30.00% of full-term infants was higher than 6.67% of preterm infants, and the difference was statistically significant(P<0.05). Conclusion Craniocerebral ultrasonography has high application value in the diagnosis of neonatal intracranial hemorrhage, which can better determine the situation of neonatal intracranial hemorrhage and provide imaging basis for clinical treatment.
