摘　　要：目的 探讨通窍耳聋丸、地塞米松联合治疗对突发性聋患者血液流变学、内皮功能和免疫功能的影响。方法 将2019年9月至2021年9月万宁市人民医院收治的98例突发性聋患者纳入本次前瞻性研究,采用区组随机分为对照组(n=49)和研究组(n=49)。对照组患者给予鼓室注射地塞米松治疗,研究组患者给予口服通窍耳聋丸+鼓室注射地塞米松治疗,持续治疗10 d。比较两组的临床疗效,治疗前及治疗10 d后的血液流变学(纤维蛋白原、全血高切黏度、全血低切黏度)、内皮功能[内皮素-1(ET-1)和可溶性血管细胞间黏附分子-1(sVCAM-1)]、免疫功能相关指标[CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、免疫球蛋白(Ig) A、IgG、IgM]和不良反应情况。结果 研究组的总有效率为97.96%,显著高于对照组(85.71%),差异有统计学意义(P <0.05)。研究组治疗后的纯音听阈值为(32.85±6.06)dB,明显低于对照组[(48.35±6.88) dB],差异有统计学意义(P <0.05)。研究组治疗后的纤维蛋白原、全血高切黏度、全血低切黏度分别为(2.51±0.50) g/L、(3.70±0.81) mPa·s、(6.83±1.61) mPa·s,均显著低于对照组[(3.60±0.47) g/L、(5.68±0.97) mPa·s、(9.01±1.72) mPa·s],差异均有统计学意义(P <0.05)。研究组治疗后的ET-1、sVCAM-1为(85.90±18.56) g/L、(172.38±21.92) ng/mL,均显著低于较对照组[(118.24±20.98) g/L、(232.64±23.17) ng/mL],差异均有统计学意义(P <0.05)。研究组治疗后的细胞免疫指标CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)分别为(48.05±5.19)%、(39.87±4.77)%、(1.70±0.22),均显著高于对照组[(44.10±5.24)%、(36.12±5.01)%、(1.35±0.20)%],而CD8^(+)为(23.17±2.23)%,显著低于对照组[(26.08±2.00)%],差异均有统计学意义(P <0.05)。研究组治疗后的体液免疫指标IgA、IgG含量分别为(1.99±0.18)、(10.21±0.90) g/L,均显著低于对照组[(2.49±0.20)、(12.40±0.95) g/L],差异均有统计学意义(P <0.05)。两组患者的不良反应发生率�Objective To investigate the effects of Tongqiao Erlong pills combination with dexamethasone on hemorheology,endothelial function and immune function in patients with sudden deafness.Methods A total of 98 patients with sudden deafness treated in Wanning People's Hospital from September 2019 to September 2021 were randomly divided into control group(n=49)and study group(n=49).The control group was treated with intratympanic injection of dexamethasone,and the study group was treated with oral Tongqiao Erlong pills+intratympanic injection of dexamethasone.The treatment last for 10 days.The clinical efficacy,hemorheology(including fibrinogen,whole blood high shear viscosity and whole blood low shear viscosity),endothelial function[endothelin-1(ET-1)and soluble vascular intercellular adhesion molecule-1(sVCAM-1)],immune function indexes[CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+),immunoglobulin(Ig)A,IgG,IgM]before and 10 days after treatment and adverse reactions were compared.Results The total effective rate of the study group was 97.96%,which was significantly higher than that of the control group(85.71%),and the difference was statistically significant(P<0.05).The pure tone hearing threshold of study group after treatment was(32.85±6.06)dB,which was lower than that of the control group[(48.35±6.88)dB],and the difference was statistically significant(P<0.05).After treatment,the fibrinogen,whole blood high shear viscosity and whole blood low shear viscosity in the study group were(2.51±0.50)g/L,(3.70±0.81)mPa·s,(6.83±1.61)mPa·s,respectively,which were lower than those in the control group[(3.60±0.47)g/L,(5.68±0.97)mPa·s,(9.01±1.72)mPa·s],the differences were statistically significant(P<0.05).After treatment,ET-1 and sVCAM-1 in study group were(85.90±18.56)g/L,(172.38±21.92)ng/mL,respectively,which were lower than those in control group[(118.24±20.98)g/L,(232.64±23.17)ng/mL],the differences were statistically significant(P<0.05).After treatment,CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in study group were(48.05±5
