PCOS患者血清IGF-1、CTRP9与生殖激素及代谢水平变化的关系

摘　　要：目的 探讨多囊卵巢综合征(PCOS)患者血清胰岛素样生长因子-1(IGF-1)、C1q/肿瘤坏死因子相关蛋白9(CTRP9)水平与患者生殖激素水平及代谢水平变化的关系。方法 采用病例对照研究方法,选取2019年10月至2021年12月海南医学院第一附属医院确诊的PCOS患者98例(PCOS组),选取健康妇女100名作为对照组。检测对比两组的血清IGF-1、CTRP9、血脂[总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]、血糖代谢指标[空腹血糖(FPG)、餐后2 h血糖(2 h PG)、空腹胰岛素(FINS)、稳态模型胰岛素抵抗指数(HOMA-IR)]、生殖激素[促黄体生成素(LH)、促卵泡激素(FSH)、雌激素(E2)、睾酮]水平;比较不同体重指数(BMI)水平的PCOS组患者IGF-1、CTRP9水平;并采用线性相关分析法探讨血清IGF-1、CTRP9与其余各项指标的相关性。结果 PCOS组患者的血清IGF-1水平为(136. 4±18. 3)μg/L、高于对照组[(98. 1±11. 5)μg/L],CTRP9水平为(254. 8±43. 7) ng/mL,低于对照组[(438. 1±96. 1) ng/mL],差异均有统计学意义(P<0.05)。PCOS组患者的血清HDL-C水平低于对照组,TC、TG、LDL-C、FPG、2 h PG、FINS、HOMA-IR、睾酮、LH、E2水平高于对照组,差异均有统计学意义(P<0.05)。BMI> 25. 0 kg/m2的PCOS组患者的血清IGF-1水平大于BMI≤25. 0 kg/m2的PCOS组患者,CTRP9水平低于BMI≤25. 0 kg/m2的PCOS组患者,差异均有统计学意义(P<0.05)。PCOS组患者的血清CTRP9水平与TC、TG、LDL-C、2 hPG、HOMA-IR、睾酮值均呈负相关(P<0.05),IGF-1水平与TC、LDL-C、FPG、2 hPG、HOMA-IR水平均呈正相关(P<0.05)。结论 PCOS患者血清IGF-1水平升高、CTRP9水平降低,IGF-1水平异常主要与患者代谢指标变化有关,CTRP9水平变化与患者代谢水平及血清雄激素睾酮变化有关。Objective To investigate the relationship between the changes of serum insulin-like growth factor-1(IGF-1)and C1q/tumor necrosis factor-related protein 9(CTRP9)levels in patients with polycystic ovary syndrome(PCOS)and the changes of reproductive hormone levels and metabolic levels.Methods In this study,a case-control study method was used to select 98 patients with PCOS(PCOS group)diagnosed in the First Affiliated Hospital of Hainan Medical University from October 2019 to December 2021,and 100 healthy women as the control group.The levels of CTRP9,blood lipid[total cholesterol(TC),triglycerides(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)]and blood glucose[fasting blood glucose(FPG),2 h postprandial blood glucose(2 hPG),fasting insulin(FINS),homeostasis model insulin resistance index(HOMA-IR)],metabolism indexes[luteinizing hormone(LH),follicle stimulating hormone(FSH),estrogen(E2),testosterone],and reproductive hormones.The levels of IGF-1 and CTRP9 in PCOS patients with different body mass index(BMI)levels were compared.And the correlation between serum IGF-1,CTRP9 and other indexes was investigated by linear correlation analysis.Results The level of serum IGF-1 in the PCOS group was(136.4±18.3)μg/L,which was higher than that in the control group[(98.1±11.5)μg/L],and the level of CTRP9 in the PCOS group was(254.8±43.7)ng/mL,which was lower than that in the control group[(438.1±96.1)ng/mL],and the differences were statistically significant(P<0.05).The level of HDL-C in the PCOS group was lower than that in the control group,the levels of TC,TG,LDL-C,FPG,2 hPG,FINS,HOMA-IR,testosterone,LH,and E2 in the PCOS group were higher than those in the control group,and the differences were statistically significant(P<0.05).The level of IGF-1 was higher than that in the PCOS group with BMI value≤25.0 kg/m 2,the level of CTRP9 in the PCOS group with BMI value>25.0 kg/m 2 was lower than that in the PCOS group with BMI value≤25.0 kg/m 2,and and the differences were stat
