苓桂术甘汤联合温针灸治疗慢性心力衰竭的疗效及对患者心功能和血管内皮功能的影响

摘　　要：目的观察苓桂术甘汤联合温针灸治疗慢性心力衰竭(CHF)的疗效,并探讨其对患者心功能和血管内皮功能的影响。方法选取2019年1月至2021年1月上海市第十人民医院中医科收治的100例CHF患者为研究对象,采用随机数表法分为观察组和对照组各50例。对照组患者给予西医治疗,观察组患者在对照组治疗的基础上联合苓桂术甘汤+温针灸治疗,均治疗14 d。治疗14 d后,比较两组的治疗效果,统计治疗前后的心功能[左室收缩末期内径(LVESD)、左心室射血分数(LVEF)]、血管内皮功能[一氧化氮(NO)、内皮素-1(ET-1)、B型钠尿肽(BNP)]变化和不良反应发生情况。结果观察组患者的治疗总有效率为94.00%,明显高于对照组的78.00%,差异具有统计学意义(P<0.05);治疗后,两组患者的LVESD降低,且观察组为(41.14±5.01)mm,明显低于对照组的(48.65±4.81)mm,而LVEF升高,且观察组为(53.41±4.18)%,明显高于对照组的(47.18±4.30)%,差异均具有统计学意义(P<0.05);治疗后,两组患者的BNP、ET-1均低于治疗前,且观察组分别为(0.56±0.11)μg/L、(53.14±4.09)ng/L,明显低于对照组的(0.89±0.17)μg/L、(67.41±4.13)ng/L,而NO升高,且观察组为(132.98±12.45)μmol/L,明显高于对照组的(109.31±10.71)μmol/L,差异均有统计学意义(P<0.05);两组患者治疗期间的不良反应比较差异无统计学意义(P>0.05)。结论苓桂术甘汤联合温针灸治疗CHF可有效改善患者的心室功能及血管内皮功能,临床治疗效果显著,且安全性高。Objective To observe the curative effect of Linggui Zhugan decoction combined with warm acupuncture in the treatment of chronic heart failure(CHF),and to explore its effect on cardiac function and vascular endothelial function.Methods A total of 100 patients with CHF admitted in Department of Traditional Chinese Medicine,Shanghai Tenth People's Hospital from January 2019 to January 2021 were selected as the research objects.They were randomly divided into an observation group and a control group,with 50 patients in each group.The patients in the control group were treated with western medicine,and the patients in the observation group were treated with Linggui Zhugan Decoction combined with warm acupuncture on the basis of the the control group,all for 14 days.After 14 days of treatment,the therapeutic effects of asthma in the two groups were compared,and the changes of cardiac function[left ventricular end systolic diameter(LVESD),left ventricular ejection fraction(LVEF)],vascular endothelial function[nitric oxide(NO),endothelin-1(ET-1),B-type natriuretic peptide(BNP)],and adverse reactions before and after treatment were counted.Results The total effective rate of the observation group was 94.00%,which was significantly higher than 78.00%of the control group(P<0.05).After treatment,LVESD level of the two groups decreased,and the LVESD in the observation group was(41.14±5.01)mm,which was significantly lower than(48.65±4.81)mm in the control group;the LVEF level increased,and LVEF level in the observation group was(53.41±4.18)%,significantly higher than(47.18±4.30)%in the control group(P<0.05).After treatment,the the levels of FT-1 and BNP of the two groups were lower than those before treatment,and the levels of BNP and ET-1 in the observation group were(0.56±0.11)μg/L and(53.14±4.09)ng/L,which were significantly lower than(0.89±0.17)μg/L and(67.41±4.13)ng/L in the control group;the level of NO increased,and the level of NO in the observation group was(132.98±12.45)μmol/L,which was significantly high
