超声引导下高渗葡萄糖增生疗法治疗冈上肌腱病的临床价值

摘　　要：目的探讨超声引导下高渗葡萄糖疗法治疗冈上肌腱腱病的临床价值。方法选取2018年1月至2021年10月期间在南通市第三人民医院及南通大学附属医院就诊,磁共振确诊为冈上肌腱腱病的30例患者,超声引导下在腱病处注射15%的葡萄糖,合并肩周炎者超声引导下在肩周间隙内注射灭菌注射用水,合并继发性冻结肩者行超声引导下灭菌注射用水注射扩张肩关节腔,治疗前后均行肩关节超声检查及肩关节Constant评分评估。结果30例患者治疗后腱病处边界模糊,面积较治疗前下降但差异无统计学意义(P>0.05);14例合并继发性冻结肩患者治疗后肩关节囊厚度减少,与治疗前比较差异有统计学意义(P<0.05);能量多普勒显示4例腱病内部出现散在点状血流信号;30例患者治疗前的Constant评分为(60.63±12.41)分,治疗3个月复查提高到(95.20±2.24)分,差异有统计学意义(P<0.05);22例合并肩周炎症患者治疗后炎症症状消失。结论超声引导下高渗葡萄糖增生疗法治疗冈上肌腱病具有较高的临床应用价值。Objective To investigate the clinical value of ultrasound-guided hyperosmotic glucose hyperplasia therapy in the treatment of supraspinatus tendon disease.Methods Thirty patients with supraspinatus tendon disease diagnosed by MRI in Nantong Third People's Hospital and the Affiliated Hospital of Nantong University from January 2018 to October 2021 were injected with 15%glucose at the tendon disease under the guidance of ultrasound.Besides,sterilized water for injection was injected into the perishoulder space under the guidance of ultrasound for the patients complicated with scapulohumeral periarthritis,and the patients complicated with secondary frozen shoulder was injected with sterilized water to expand the shoulder joint cavity.Shoulder joint ultrasonography and shoulder joint Constant score evaluation were performed before and after treatment.Results After treatment,the boundary of 30 patients with tendon disease was blurred,and the area decreased slightly compared with that before treatment,with was no statistically significant difference(P>0.05).The thickness of shoulder joint capsule decreased in 14 patients with secondary frozen shoulder after treatment,which showed statistically significant difference compared with that before treatment(P<0.05).Power Doppler showed scattered punctate blood flow signals in 4 cases of tendinopathy.The constant score of 30 patients before treatment was(60.63±12.41)points,which was increased to(95.20±2.24)points after three months(P<0.05).The inflammatory symptoms of 22 patients complicated with scapulohumeral periarthritis disappeared after treatment.Conclusion Ultrasound-guided hyperosmotic glucose hyperplasia therapy has high clinical value in the treatment of supraspinatus tendon disease.
