摘 要:探讨葡萄球菌A蛋白(SPA)快速检验对金黄色葡萄球菌(SA)的临床监测的意义。方法:收集广东省中医院临床分离的SA菌株(108株)与非SA菌株(77株)共185株,均进行SPA镜下快速检测,分析108株SA的耐药性及快速检测结果。另选取2021年1月-2022年1月于广东省中医院收集的ICU患者的痰液标本80份,对其进行痰培养鉴定及SPA快速检测,比较两种检测方法的结果。结果:108株SA中甲氧西林敏感SA(MSSA)与甲氧西林耐药SA(MRSA)的占比分别为46.30%、53.70%,MSSA中以青霉素G(72.00%)耐药率较高,MRSA中以青霉素G(91.38%)、克林霉素(75.86%)、左氧氟沙星(62.07%)耐药率较高。108株SA经SPA快速检测的灵敏度、特异度、准确度均为100.00%。以痰培养鉴定为标准,SPA快速诊断SA的灵敏度、特异度、准确度、Kappa值分别为100.00%、96.55%、97.50%、0.939。结论:SA耐药性较强,通过SPA快速检测可用于早期SA感染的快速诊断,其诊断结果较为可靠。Objective: To explore the significance of rapid detection of Staphylococcus A protein (SPA) on clinical monitoring of Staphylococcus aureus (SA). Methods: A total of 185 SA strains (108 strains) and non-SA strains (77 strains) were collected from Guangdong Provincial Hospital of Chinese Medicine. They were subject to rapid detection of SPA under microscope. The drug resistance and rapid detection results of 108 SA strains were analyzed. In addition, 80 sputum samples from ICU patients collected in Guangdong Provincial Hospital of Chinese Medicine from January 2021 to January 2022 were selected for sputum culture identification and SPA rapid detection, and the results of the two detection methods were compared. Results: Among 108 strains of SA, the proportion of methicillin sensitive SA (MSSA) and methicillin resistant SA (MRSA) were 46.30% and 53.70% respectively. The resistance rate of penicillin G (72.00%) was higher in MSSA, and that of penicillin G (91.38%), clindamycin (75.86%), levofloxacin (62.07%) was higher in MRSA. The sensitivity, specificity and accuracy of the SPA rapid detection of 108 strains of SA were 100.00%. Taking the sputum culture identification as the standard, the sensitivity, specificity, accuracy and Kappa value of SPA for rapid diagnosis of SA were 100.00%, 96.55%, 97.50% and 0.939 respectively. Conclusion: SA has strong drug resistance. SPA rapid detection can be used for rapid diagnosis of early SA infection, and its diagnostic results are reliable.
