蛇毒血凝酶联合缩宫素治疗高龄产妇剖宫产术后出血的临床研究

摘　　要：目的 探讨蛇毒血凝酶联合缩宫素治疗高龄产妇剖宫产术后出血的临床价值。方法 选取2018年5月至2020年5月本院妇产科收治的120例剖宫产产后出血(PPH)高龄产妇作为研究对象,随机分为研究组(n=62)与对照组(n=58)。对照组宫体给予缩宫素和葡萄糖酸钙,研究组在对照组基础上联用蛇毒血凝酶。比较两组治疗期间失血量、初步止血时间、红细胞输入量、止血治疗结束时血红蛋白水平、临床疗效及不良反应发生率。结果 研究组治疗期间失血量、红细胞输入量均少于对照组,初步止血时间短于对照组,结束治疗时血红蛋白水平高于对照组(P<0.05);研究组治疗总有效率为95.16%,高于对照组的77.59%(P<0.05);研究组不良反应发生率为11.29%,低于对照组的25.86%(P<0.05)。结论 蛇毒血凝酶联合缩宫素治疗高龄产妇剖宫产术后出血的疗效显著,可有效促进产妇子宫收缩,加快失血部位凝血进程,有效改善高龄产妇术后失血症状并降低失血量,且安全性高,值得临床推广应用。Objective To investigate the clinical value of snake venom hemagglutinin combined with oxytocin in the treatment of bleeding after cesarean section in elderly puerperae.Methods 120 cases of postpartum hemorrhage(PPH) in elderly women after cesarean section who were admitted to the Obstetrics and Gynecology Department of our hospital from May 2018 to May 2020 were selected as the research subjects,and they were randomly divided into the study group(n=62) and the control group(n=58) according to the random number table method.The control group was given oxytocin and calcium gluconate in the uterus,and the study group was given snake venom hemagglutinase on the basis of the control group.The blood loss during the treatment,initial hemostasis time,the red blood cell input,the hemoglobin level at the end of hemostatic treatment,clinical efficacy and incidence of adverse reactions were compared between the two groups.Results The blood loss during the treatment and red blood cell input in the study group were less than those in the control group,the initial hemostasis time was shorter than that in the control group,and the hemoglobin level at the end of treatment was higher than that in the control group(P<0.05).The total effective rate of treatment in the study group was 95.16%,which was higher than 77.59% in the control group(P<0.05).The incidence of adverse reactions in the study group was 11.29%,which was lower than 25.86% in the control group(P<0.05).Conclusion The clinical efficacy of snake venom hemagglutinase combined with oxytocin in the treatment of post-cesarean section in advanced-age PPH women is significant,it can effectively promote uterine contractions,accelerate the coagulation process at the blood loss site,effectively improve the symptoms of post-operative blood loss and reduce the blood loss in elderly women and with high safety,it is worthy of clinical promotion and application.
