腹腔镜胆囊切除术患者行全身麻醉联合超声引导下腹横肌平面阻滞的效果

摘　　要：目的:探究全身麻醉联合超声引导下腹横肌平面阻滞麻醉方案在腹腔镜胆囊切除术中的效果。方法:选择腹腔镜胆囊切除术患者作为研究对象,共74例,其中37例采取全身麻醉为对照组,剩余37例麻醉方案选择全身麻醉、超声引导下腹横肌平面阻滞(试验组)。比较两组麻醉效果、麻醉恢复情况、并发症发生率。结果:两组患者MAP、HR、SPO_(2)在T_(0)时刻比较,差异无统计学意义(P>0.05)。T_(1)、T_(2)、T_(3)时刻试验组MAP、HR均低于T_(0),且试验组低于对照组,差异有统计学意义(P<0.05)。SPO_(2)在手术全程变化不明显,两组比较差异无统计学意义(P>0.05)。试验组患者麻醉苏醒期拔管时间、睁眼时间、恢复自主呼吸时间均短于对照组,差异有统计学意义(P<0.05)。试验组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:腹腔镜胆囊切除术中对患者进行全身麻醉联合超声引导下腹横肌平面阻滞,可获得理想的麻醉效果,且麻醉恢复快、恢复良好,值得推广使用。Objective:To explore the application effect of general anesthesia combined with ultrasound-guided transversus abdominis plane block in laparoscopic cholecystectomy.Methods:Patients with laparoscopic were selected as the research subjects,with 74 cases,among them,37 cases received general anesthesia as the control group,and the remaining 37 cases received general anesthesia combined with ultrasound-guided plane block.The effect of anesthesia,the recovery of anesthesia,and the were compared.Results:There was no significant difference in MAP,HR and SPO_(2) of patients at T_(0) time(P>0.05).T_(1),T_(2),T_(3),time group MAP,HR were lower than T_(0),and the test group was lower than the control group,with statistical significance(P<0.05).There was no significant change in SPO_(2) during the whole operation(P>0.05).The extubation time,eye opening time,and recovery time of spontaneous breathing in the experimental group were shorter than those in the control group during the recovery period from anesthesia,with statistical significance(P<0.05).The incidence of complications in the experimentalwas lower than that in the control group,with statistical significance(P<0.05).Conclusion:The application of general anesthesia combined with ultrasound-guided transversus abdominis plane block in laparoscopic cholecystectomy can achieve an ideal anesthesia effect,and the recovery from anesthesia is fast and good,which is worthy of popularization.
