摘　　要：目的:分析枸橼酸氯米芬联合二甲双胍治疗肥胖型多囊卵巢综合征(PCOS)对患者性激素及糖脂代谢的影响。方法:选取2018年1月-2020年1月本院收治的肥胖型PCOS患者164例,随机分为观察组(n=82)与对照组(n=82),均从月经周期第5d给予口服枸橼酸氯米芬胶囊治疗,观察组联合口服二甲双胍。观察两组治疗前后性激素及糖脂代谢指标变化。结果:治疗后两组性激素水平,卵泡刺激素、黄体生成素、睾酮、雄烯二酮、雌二醇水平均明显降低且观察组减低幅度大于对照组,两组空腹血糖、空腹胰岛素水平,胰岛素抵抗指数均显著低于治疗前且观察组降低幅度大于对照组,两组甘油三酯、总胆固醇、低密度脂蛋白水平均低于治疗前,高密度脂蛋白水平高于治疗前,且观察组变化幅度优于对照组(均P<0.05)。结论:枸橼酸氯米芬联合二甲双胍可有效降低肥胖型PCOS患者性激素水平,改善机体糖脂代谢状态,效果确切。Objective: To analyze the effects of clomiphene citrate combined with metformin for treating obese patients with polycystic ovary syndrome(PCOS) on their sex hormones levels and glucolipid metabolism. Methods: 164 obese patients with PCOS were selected and were randomly divided into two groups from January 2018 to January 2020. 82 patients in the observation group were treated by clomiphene citrate capsule combined with metformin on the 5 th day of their menstrual cycle, and 82 patients in the control group were treated by clomiphene citrate capsule only on the 5 th day of their menstrual cycle. The changes of sex hormones levels and glucolipid metabolism indexes of the patients in the two groups before and after treatment were observed. Results: The levels of sex hormones, such as follicle stimulating hormone, luteinizing hormone, testosterone, androstenedione, and estradiol, of the patients in the two groups after treatment had decreased significantly, and the reduction extents of which of the patients in the observation group were significantly more than those of the patients in the control group. The levels of fasting blood glucose and fasting insulin, and the values of insulin resistance index of the patients in the two groups after treatment had decreased significantly, and the reduction extents of which of the patients in the observation group were significantly more than those of the patients in the control group. The levels of triglyceride, total cholesterol, and low density lipoprotein of the patients in the two groups after treatment had decreased significantly, while the level of high density lipoprotein of the patients in the two groups after treatment had increased significantly, and the rangeability of which of the patients in the observation group were significantly better than those of the patients in the control group(all P<0.05).
