摘 要:目的:探讨子痫前期患者外周血和胎盘组织中双调蛋白(amphiregulin)表达水平及与妊娠结局的关系。方法:选取2019年6月-2020年9月本院收治的子痫前期孕妇203例临床资料,分为子痫前期轻度组(102例)和子痫前期重度组(101例),产前检查健康孕妇108例为对照组。采用实时荧光逆转录法(RT-PCR)、酶联免疫吸附法(ELISA)、免疫组化法测定外周血和胎盘组织中Amphiregulin mRNA和蛋白表达水平,记录孕妇妊娠结局(胎盘重量、新生儿出生体重、1min Apgar评分)。比较各组孕妇外周血和胎盘组织中Amphiregulin表达差异并分析与妊娠结局关系。结果:子痫前期重度组、子痫前期轻度组、对照组,血清和胎盘Amphiregulin mRNA和蛋白表达水平、Amphiregulin阳性率均依次升高,胎盘重量、新生儿出生体重、1min Apgar评分均依次升高,血清及胎盘Amphiregulin mRNA和蛋白与胎盘重量、新生儿出生体重、1min Apgar评分均呈正相关(均P<0.05)。结论:子痫前期患者外周血和胎盘组织中Amphiregulin表达水平异常降低,且随病情加重降低;Amphiregulin与子痫前期严重程度呈负相关,与不良妊娠结局正相关。提示临床,Amphiregulin有望作为评估子痫前期患者妊娠结局的生物学标志物。Objective: To investigate the expression level of Amphiregulin in peripheral blood and placenta tissue of patients with preeclampsia and its relationship with their pregnancy outcomes. Methods: A total of 203 pregnant women with preeclampsia were selected as the research objects from June 2019 to September 2020. According to the severity of preeclampsia, these women were divided into group A(102 cases with mild preeclampsia) and group B(101 cases with severe preeclampsia). 108 healthy pregnant women were selected in group C. Real-time fluorescence reverse transcription(RT-PCR), enzyme-linked immunosorbent assay(ELISA), and immunohistochemistry were used to determine the expression levels of Amphiregulin mRNA and protein in peripheral blood and placental tissues of the women in the three groups. The pregnancy outcomes(placental weight, newborn birth, body weight, 1 min Apgar score) of the women in the three groups were recorded. The differences of the expression of Amphiregulin in peripheral blood and placental tissues of the women were compared among the three groups. The relationship between the expression of Amphiregulin in peripheral blood and placental tissues of the women and their pregnancy outcomes was analyzed. Results: The expression levels of the serum and placental Amphiregulin mRNA and protein, the positive rate of Amphiregulin, the placental weight, the newborn birth weight, and the newborn 1 min Apgar score of the women in group B, group A, and group C had increased gradually. The expression levels the mRNA and protein of serum and placental Amphiregulin of the women were positively correlated with their placental weight, newborn birth weight, and 1 min Apgar score(all P<0.05). Conclusion: The expression levels the mRNA and protein of serum and placental Amphiregulin of the women with preeclampsia decrease abnormally, and which will decrease with the severity of preeclampsia.The expression levels the mRNA and protein of serum and placental of the women are negatively correlated with the severity of
