宫腔镜联合钳夹术或电切术清除胚物残留临床效果比较

摘　　要：目的:比较宫腔镜联合钳夹术或宫腔镜电切术治疗胚物残留的临床效果。方法:回顾性分析2018年1月—2021年12月本院行不全流产胚物残留宫腔镜手术175例资料,分为电切组(n=71)和钳夹组(n=104),比较两组术中术后情况。结果:两组手术时间、术中出血量、术后阴道流血时间、术中发现宫腔粘连、子宫严重屈曲及宫腔畸形发生率无差异(P>0.05);两组并发症发生率分别为2.8%、4.8%;一次性完全清除率分别为95.8%、97.1%;术后新发宫腔粘连率分别为2.8%、1.9%均无差异(P>0.05)。结论:宫腔镜联合钳夹术治疗胚物残留临床效果与宫腔镜电切术相当,是适合基层医院开展的简易、安全、高效的手术方式。Objective: To compare the clinical effect of hysteroscopic electrotomy and hysteroscopic clamp for removing the retained products of conception(RPOC). Methods: The data of 175 patients who underwent hysteroscopic RPOC removal from January 2018 to December 2021 were analyzed retrospectively. These patients were divided into group A(71 cases with hysteroscopic electrotomy for removing RPOC) and group B(104 cases with hysteroscopic hysteroscopic clamp for removing RPOC). The situations during operation of the patients were compared between two groups. Results: There were no significant differences in the operative time, the intraoperative blood loss, the postoperative vaginal bleeding time, the incidences of intrauterine adhesions, severe uterine flexion, and uterine malformation of the patients between the two groups(P>0.05). There were no significant differences in the incidence of complications(2.8% vs. 4.8%), the complete removal rate(95.8% vs. 97.1%), and the rate of intrauterine readhesion after operation(2.8% vs. 1.9%) of the patients between the two groups(P>0.05). Conclusion: The clinical effect of hysteroscopic clamp for removing RPOC of the patients is similar to that of hysteroscopic electrotomy, which is a feasible operative method with the characteristics of simple, effectiveness, and safety, especially suitable for the primary hospital.
