宫颈癌多普勒超声血流参数与患者血清MMP-2表达的相关性

摘　　要：目的:检测宫颈癌患者多普勒超声血流参数与血清基质金属蛋白酶-2(MMP-2)的表达情况,探讨其相关性及诊断价值。方法:选取2016年10月-2019年10月于本院经病理证实为宫颈癌患者83例为宫颈癌组,宫颈癌上皮瘤变(CIN)患者70例为CIN组,子宫良性病变患者70例为对照组,均行多普勒超声检查得到收缩期峰值血流速度(PSV)与阻力指数(RI);酶联免疫吸附法法检测血清MMP-2水平;将宫颈癌组患者根据FIGO分期分为I期组29例、II期组25例、III期组16例,比较3组PSV、RI及血清MMP-2水平;Pearson法分析宫颈癌患者血清MMP-2水平与PSV、RI的相关性;受术者工作特征曲线(ROC)分析PSV、RI与血清MMP-2水平诊断宫颈癌效能。结果:对照组、CIN组、宫颈癌组患者PSV、MMP-2水平依次升高,RI水平依次降低,且宫颈癌组随FIGO分期的增大患者PSV、血清MMP-2水平逐渐增大,RI水平逐渐减小(均P<0.05)。宫颈癌患者血清MMP-2与PSV呈正相关(r=0.690,P<0.05),与RI呈负相关(r=-0.516,P<0.05)。ROC分析显示,诊断宫颈癌的曲线下面积PSV为0.783,RI为0.777,PSV与RI联合为0.864,血清MMP-2为0.883,血清MMP-2诊断价值高于PSV和RI单独检测(P<0.05),但与PSV、 RI联合诊断价值无差异(P>0.05)。结论:宫颈癌患者PSV、血清MMP-2升高,RI降低,超声PSV、RI联合检测与血清MMP-2检测对宫颈癌的诊断价值相当,提示MMP-2可在一定程度替代多普勒超声诊断,有望作为鉴别宫颈癌与癌前病变的生物学指标。Objective: To detect the values of blood flow parameters of cervical cancer tissue by Doppler ultrasound and the expression level of matrix metalloproteinase-2(MMP-2) of the patients, to study the correlation between the values of blood flow parameters of cervical cancer tissue of the patients and their serum MMP-2 level, and to analyze their value for diagnosing cervical cancer. Methods: A total of 83 patients with cervical cancer confirmed by pathology were selected in group A, and 70 patients with cervical cancer epithelial neoplasia(CIN) were selected in group B from October 2016 to October 2019. Meanwhile, 70 patients with benign uterine lesions were selected in group C. The values of peak systolic velocity(PSV) and resistance index(RI) of lesion tissue of the patients in the three groups were examined by Doppler ultrasound. The level of serum MMP-2 of the patients in the three groups was detected by enzyme-linked immunosorbent assay. The patients in group A were further divided into group A1(29 cases with stage I), group A2(25 cases with stage II), and group A3(16 cases with stage III) according to FIGO stage of cervical cancer, and the value of PSV and RI, and the serum MMP-2 level of the patients were compared among the three groups. Pearson method was used to analyze the correlation between the serum MMP-2 level of the patients with cervical cancer and their PSV and RI values. Receiver operator characteristic(ROC) curve was used to analyze the diagnostic efficacy of the PSV and RI values, and the serum MMP-2 level of the patients for their cervical cancer. Results: The PSV value and MMP-2 level of the p atients in group C,group B,and group A had increased gradually,while the value of RI of the patients had decreased gradually.The PSV value and MMP-2 level of the patients in group A1,group A2,and group A3 had increased gradually,while the value of RI of the patients had decreased gradually(all P<0.05).The serum MMP-2 level of the patients with cervical cancer was positively correlated with their PSV value
