六君消瘰汤加减结合雷火灸治疗小儿腺样体肥大(肺脾气虚证)临床疗效及安全性观察

摘　　要：目的探究六君消瘰汤加减结合雷火灸治疗小儿腺样体肥大(肺脾气虚证)临床疗效及安全性。方法研究纳入92例腺样体肥大(肺脾气虚证)患儿,均由医院门诊2020年11月—2021年3月收治,以随机数字表法分成两组,予以对照组46例患儿孟鲁司特钠+糠酸莫米松鼻喷雾剂治疗,予以观察组46例患儿孟鲁司特钠及糠酸莫米松鼻喷雾剂+雷火灸与六君消瘰汤加减结合治疗,比较患儿治疗后疗效、患儿治疗前后中医证候积分(鼻塞打鼾、张口呼吸、胸闷憋气、呼吸反复暂停、嗜睡等)变化情况、特异性变应原IgE(sIgE)水平变化、治疗前后患儿电子鼻咽喉镜评分变化及睡眠障碍评分变化、两组患儿治疗不良反应情况。结果与观察组患儿(95.65%,44/46)相比,对照组患儿治疗总有效率(82.61%,38/46)更低(P<0.05);两组患儿治疗前中医证候积分(鼻塞打鼾、张口呼吸、胸闷憋气、呼吸反复暂停、嗜睡等)、sIgE水平、电子鼻咽喉镜评分及睡眠障碍评分比较,P>0.05,两组患儿经临床治疗后中医证候积分(鼻塞打鼾、张口呼吸、胸闷憋气、呼吸反复暂停、嗜睡等)、sIgE水平、电子鼻咽喉镜评分及睡眠障碍评分均显著改善,而观察组患儿经治疗后中医证候积分(鼻塞打鼾、张口呼吸、胸闷憋气、呼吸反复暂停、嗜睡等)、sIgE水平、电子鼻咽喉镜评分及睡眠障碍评分优于对照组患儿(P<0.05);两组不良反应均较少,观察组与对照组分别为2例(4.35%,2/46)、3例(6.52%,3/46),P>0.05。结论六君消瘰汤加减结合雷火灸治疗小儿腺样体肥大(肺脾气虚证)临床疗效显著,患儿恢复好,不良反应少,安全可靠,值得临床推广与应用。Objective To explore the clinical efficacy and safety of Modified Liujun Xiaolei Decoction(六君消瘰汤加减)combined with thunder fire moxibustion in the treatment of adenoid hypertrophy(lung-spleen Qi deficiency syndrome)in children.Methods Ninety-two children with adenoid hyperplasia(lung-spleen Qi deficiency syndrome)were enrolled in this study.They were admitted to the outpatient department of the hospital from November 2020 to March 2021 and divided into two groups by random number table method.Forty-six children in the control group were treated with montelukast sodium+momethasone furoate nasal spray.Forty-six children in the observation group were treated with montelukast sodium and momethasone furoate nasal spray+thunder fire moxibustion combined with Modified Liujun Xiaolei Decoction.The curative effect of the children after treatment,the changes of TCM syndrome scores(nasal congestion and snoring,mouth breathing,chest tightness and suffocation,recurrent pausing of breathing,drowsiness,etc.)before and after treatment,the changes of specific allergen IgE(sIgE)level,the changes of electronic nasopharyngoscope score and sleep disorder score before and after treatment,and the adverse reactions of the two groups were compared.Results Compared with that(95.65%,44/46)of the observation group,the total effective rate(82.61%,38/46)of the control group was lower(P<0.05).Comparison of TCM syndrome scores(nasal congestion and snoring,mouth breathing,chest tightness and suffocation,recurrent pausing of breathing,drowsiness,etc.).The sIgE level,electronic nasopharyngoscopic score and sleep disorder score between the two groups before treatment were not significant(P>0.05).After clinical treatment,TCM syndrome scores(nasal congestion and snoring,mouth breathing,chest stuffiness,recurrent pausing of breathing,drowsiness,etc.),sIgE level,electronic nasolaryngoscopy score and sleep disorder score were significantly improved in both groups.After treatment,the observation group was better than the control group in TCM
