摘  要:目的研究黄芪箭丸对糖尿病周围神经病变大鼠核转录因子-κB(nuclear factor kappa B,NF-κB)及降钙素基因相关肽(calcitonin gene-related peptide,CGRP)表达的影响。方法选取68只清洁级(SPF)雄性健康Wistar大鼠,采用随机数字表法分为正常组18只;其余50只大鼠予以糖尿病周围神经病变造模,模型成功后,参考随机数字表法分为模型组和干预组,每组25只;空白组大鼠不予任何干预,模型组仅予以0.9%生理盐水10 mL/kg灌胃,干预组给予黄芪箭丸灌胃,1次/d、连续12周。观察并比较3组大鼠研究干预前后空腹血糖值变化、神经传导速度、NF-κB蛋白表达及CGRP表达。结果干预后较干预前,空白组、模型组大鼠血糖值基本无变化,差异无统计学意义(P>0.05),干预组大鼠血糖值下降,差异有统计学意义(P<0.05);干预后,空白组大鼠血糖值最低、其余依次为干预组、模型组,差异有统计学意义(P<0.05)。3组大鼠以模型组运动神经、感觉神经传导速度最低,其余依次为干预组和空白组,差异有统计学意义(P<0.05);但干预组和空白组神经传导速度接近,差异无统计学意义(P>0.05)。免疫组化显示3组大鼠以模型组积分光密度值、平均光密度值最高,其余依次为干预组和空白组,差异有统计学意义(P<0.05);干预组和空白组NF-κB蛋白表达接近,差异无统计学意义(P>0.05)。3组大鼠以干预组免疫反应阳性细胞个数最多、平均光密度值最高,其余依次为模型组和空白组,差异有统计学意义(P<0.05);模型组和空白组免疫反应阳性细胞数目、平均光密度值接近,差异无统计学意义(P>0.05)。结论黄芪箭丸通过养血活血、理气通络等补泻兼施的方式实现了“正气存内、邪不可干”的目标,切合糖尿病周围神经病变本虚标实之病机,具有较高治疗优势,并为临床提供了有效思路。Objective To study the effect of Huangqijian Pill(黄芪箭丸)on the expressions of nuclear factor kappa B(NF-κB)and calcitonin gene-related peptide(CGRP)in rats with diabetic peripheral neuropathy.Methods Sixty-eight clean-grade(SPF)male healthy Wistar rats were selected and 18 were put into the blank group by random number table method.The remaining 50 rats were modeled with diabetic peripheral neuropathy.After the model was successful,the random number table method was used to divide the rats into the model group and the intervention group,with 25 rats in each group.The rats in the blank group were not given any intervention.The model group was given 0.9%normal saline 10 mL/kg by gavage,and the intervention group was given Huangqijian Pill by gavage once a day for 12 consecutive weeks.The changes of fasting blood glucose,nerve conduction velocity,NF-κB protein expression and CGRP expression before and after intervention in the three groups were observed and compared.Results After intervention,the blood glucose level of the rats in the blank group and the model group was basically unchanged,and the difference was not statistically significant(P>0.05).The blood glucose level of the intervention group was decreased,and the difference was statistically significant(P<0.05).After intervention,the blood glucose level of the blank group was the lowest,and the rest order was the intervention group and the model group and the difference was statistically significant(P<0.05).The motor and sensory nerve conduction velocities of the rats in the model group were the lowest,and the rest order was the intervention group and the blank group and the difference was statistically significant(P<0.05).The nerve conduction velocities of the intervention group and the blank group were similar,and the difference was not statistically significant(P>0.05).Immunohistochemistry showed that the model group had the highest integrated optical density value and average optical density value among the three groups and the rest order was the
