摘 要:本文探究磁共振T_(2) mapping成像对关节镜下肩袖修补术后疗效评估价值。选取行关节镜下肩袖修补术的72例肩袖损伤体操运动员,于术前、术后6个月均行肩关节常规MR序列、T_(2) mapping序列检查,根据术后6个月疗效分为优良组(n=52)、不良组(n=20)。结果显示,优良组MRI低分级占比高于不良组,术前、术后6个月冈上肌腱中间、内侧、外侧T_(2)值低于不良组,肩关节评分量表(CMS)评分、美国加州大学肩关节评分(UCLA)高于不良组(P<0.05);患者术前、术后6个月冈上肌腱中间、内侧、外侧T_(2)值与CMS、UCLA评分均呈负相关(P<0.05);冈上肌腱中间、内侧、外侧术前、术后6个月T_(2)值与疗效显著相关(P<0.05);术后6个月冈上肌腱中间、内侧、外侧T_(2)值联合评估患者术后疗效不良的曲线下面积(AUC)最大,为0.887。磁共振T_(2) mapping成像对关节镜下肩袖修补术后疗效评估价值较高。This paper explored the efficacy value of magnetic resonance T_(2) mapping imaging in arthroscopic rotator cuff repair.72 gymnasts with rotator cuff injury underwent arthroscopic rotator cuff repair were selected,and underwent routine MR sequence and T_(2) mapping sequence examination of the shoulder joint before and 6 months after surgery.And then divided them into excellent group(n=52)and poor group(n=20)according 6 months postoperative effect.The results showed that the proportion of low-grade MRI in the excellent group was higher than in the poor group,and the T_(2) values of the supraspinatus tendon in the middle,medial and lateral sides of the supraspinatus tendon before and 6 months after surgery were lower than those in the poor group.The constant-murley score(CMS),university shoulder score(UCLA)was higher than that of the poor group(P<0.05).The T_(2) values of the middle,medial and lateral supraspinatus tendon were negatively correlated with the CMS and UCLA scores before and 6 months after surgery(P<0.05).T_(2) values of the supraspinatus tendon before and 6 months after surgery were significantly correlated with the curative effect(P<0.05).The middle,medial and lateral T_(2) values of the supraspinatus tendon at 6 months after surgery were combined to evaluate the postoperative curative effect.The poor area under the curve(AUC)was the largest at 0.887.Magnetic resonance T_(2) mapping imaging has a high value in evaluating the efficacy of arthroscopic rotator cuff repair.
