断指再植术后再植指总主动活动度恢复情况的影响因素

摘　　要：目的:探究影响断指再植术后再植指总主动活动度恢复情况的因素。方法:选取收治的行断指再植术患者226例,根据再植指总主动活动度优良情况评估分为A组(再植指总主动活动度优)106例,B组(再植指总主动活动度良)85例,C组(再植指总主动活动度差)35例。对影响患者术后再植指总主动活动度恢复的因素进行分析,并对单因素分析有差异的指标实施多因素Logistic回归分析。结果:经单因素分析结果可见,年龄、断指保存方式、离断平面、损伤机制、肌腱粘连和关节面损伤是影响患者术后的主要因素(P<0.05)。经Logistic回归分析可见,年龄、损伤机制、离断平面、肌腱粘连及关节面损伤是影响患者术后再植指总主动活动度恢复的重要因素(P<0.05)。结论:年龄、损伤机制、离断平面、关节面损伤及肌腱粘连是影响断指再植术后再植指总主动活动度恢复的主要影响因素,临床应采取针对性措施以促进患者术后指关节功能恢复。Objective To explore the factors that affect the recovery of the total active mobility of the replanted finger after replantation of the severed finger. Method A total of 226 patients undergoing amputated finger replantation were selected. According to the assessment of the total active activity of the replanted fingers, they were divided into 106 cases in group A, 85 cases in group B, and 35 cases in group C. The factors influencing the recovery of the total active activity of the patients after replantation were analyzed, and multivariate logistic regression analysis was performed on the indicators with differences in univariate analysis.Results The results of univariate analysis showed that age, preservation method of severed fingers, fracture plane, injury mechanism, tendon adhesion and articular surface injury were the main factors affecting patients after surgery(P<0.05). Logistic regression analysis showed that age, injury mechanism, fracture plane, tendon adhesion and articular surface injury were important factors affecting the recovery of the patient′s total active range of finger replantation after surgery(P<0.05). Conclusion Age, injury mechanism, fracture plane, articular surface injury, and tendon adhesion are the main influencing factors that affect the recovery of the total active range of finger replantation after replantation of severed finger. Clinical measures should be taken to promote the postoperative finger joints functional restoration.
