摘  要:目的:比较利奈唑胺与氨基糖苷类抗生素对耐甲氧西林抗药性金黄色葡萄球菌(MRSA)肺炎的疗效。方法:选取收治的MRSA肺炎患者162例为研究对象,随机分为利奈唑胺组与万古霉素组各81例。利奈唑胺组患者给予利奈唑胺治疗,万古霉素组患者给予万古霉素治疗。两组疗程均为14 d。比较两组治疗疗效、细菌清除率及治疗前后主要症状积分和炎性反应因子水平变化。结果:利奈唑胺组总有效率(96.30%)高于万古霉素组(82.72%),差异有统计学意义(P<0.05);利奈唑胺组细菌清除率(72.84%)高于万古霉素组(51.85%),差异有统计学意义(P<0.05);两组治疗后主要症状积分较治疗前降低,差异有统计学意义(P<0.05);利奈唑胺组治疗后主要症状积分低于万古霉素组,差异有统计学意义(P<0.05);两组治疗后血清CRP和TNF-α水平较治疗前降低,差异有统计学意义(P<0.05);利奈唑胺组治疗后血清CRP和TNF-α水平低于万古霉素组,差异有统计学意义(P<0.05)。结论:利奈唑胺治疗MRSA肺炎患者疗效与细菌清除率优于万古霉素,更有利于减轻炎性反应。Objective To compare the efficacy of linezoline and aminoglycoside antibiotics in the treatment of methicillin resistant Staphylococcus aureus(MRSA) pneumonia.Method 162 patients with MRSA pneumonia who were admitted to our hospital were selected as the study, they were randomly divided into linezolid group and vancomycin group. Linezolid groupwas treated with linezoline and vancomycin group with vancomycin. The course of treatment in both groups was 14 days. The therapeutic effect, bacterial clearance rate, mainsymptom score and inflammatory factor level before and after treatment were compared between the two groups. Results The total effective rate of linezolid group(96.30%) was higher than that of vancomycin group(82.72%)(P<0.05). The bacterial clearance rate of linezolid group(72.84%) was higher than that of vancomycin group(51.85%)(P<0.05). After treatment, the two groups scores of main symptoms were lower before treatment(P<0.05);after treatment, linezolid group scores of main symptoms were lowerthan vancomycin group(P<0.05). After treatment, the two groups serum CRP and TNF-α levels were lower before treatment(P<0.05);after treatment, serum CRP and TNF-α levels of linezolid groupwere lower than vancomycin group(P<0.05). Conclusion Linezoline is better than vancomycin in the treatment of MRSA, and the bacterial clearance rate is better than vancomycin, and the inflammatory response is more obvious.
