超声引导下水针刀治疗对腰椎间盘突出症患者疼痛缓解和腰椎功能恢复的影响

摘　　要：目的 探讨超声引导下水针刀治疗对腰椎间盘突出症患者疼痛缓解和腰椎功能恢复的影响。方法 按照随机数字表法将86例腰椎间盘突出症患者分成对照组和观察组,对照组采用传统水针刀疗法治疗,观察组采用超声引导下水针刀治疗,2组每周治疗1次,以2次为1个疗程。观察2组治疗前、后(1周、1个月、3个月)的视觉模拟评分法(VAS)评分、日本骨科协会(JOA)评分、Oswestry功能障碍指数(ODI)评分,比较2组治疗总有效率及安全性。结果 观察组治疗后1周、治疗后1个月及治疗后3个月的VAS评分、ODI评分均低于对照组(P<0.05),JOA评分均高于对照组(P<0.05);观察组、对照组总有效率为95.35%、79.07%,差异具有统计学意义(P<0.05);观察组不良反应发生率低于对照组(P<0.05)。结论 与传统水针刀疗法比较,超声引导下水针刀治疗腰椎间盘突出症能够更明显的缓解患者腰腿疼痛症状,改善腰椎功能,提高临床疗效及治疗安全性。Objective To investigate the effect of the ultrasound-guided injection akupotomye treatment on the pain relief and lumbar function recovery in patients with lumbar disc herniation.Methods According to the random number table method,86 patients with lumbar disc herniation were divided into a control group and an observation group.The control group was treated with the traditional injection akupotomye,while the observation group was treated with the ultrasound-guided injection akupotomye.The two groups were treated once a week,with two times as a course of treatment.The visual analog scale (VAS) score,Japanese Orthopaedic Association (JOA) score,and Oswestry Disability Index (ODI) score before and after treatment (one week,one month,and three months) in the two groups were observed,and the total effective rate and safety of the two groups were compared.Results The VAS scores and ODI scores of the observation group were lower than those in the control group one week,one month and three months after treatment (P<0.05),while the JOA scores were higher than those in the control group (P<0.05).The total effective rates in the observation group and the control group were 95.35% and 79.07% respectively,with a significant statistical difference (P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group (P<0.05).Conclusion In the treatment of lumbar disc herniation,the ultrasound-guided injection akupotomye treatment can more significantly alleviate the symptoms of low back and leg pain,improve the lumbar function,and improve the clinical efficacy and treatment safety.
