膀胱癌行膀胱全切+输尿管腹壁双造口患者术后两种造口袋护理方法效果比较

摘　　要：目的:比较膀胱癌行膀胱全切+输尿管腹壁双造口患者术后免扣造口袋护理方法与扣造口袋护理方法的效果。方法:选取2018年5月1日-2021年5月31日76例行膀胱全切+输尿管腹壁双造口术的膀胱癌患者为研究对象,根据患者选择的术后3个月护理方法分为免扣造口袋组和扣造口袋组各38例;术后3个月采用世界卫生组织生存质量测定量表简表(WHOQOL-BREF)评估比较两种护理方法患者的生存质量;3个月后采用另一种护理方法,6个月后患者自由选择最终采用何种护理方法并记录理由,分析影响因素。结果:采用免扣造口袋护理方法的患者术后3个月WHOQOL-BREF评分高于采用扣造口袋护理方法的患者(P<0.05);年龄、造口护理方式、参加造口相关活动是患者术后3个月WHOQOL-BREF评分的独立影响因素(P<0.05);术后6个月,54例患者选择免扣造口袋护理方法,22例患者选择扣造口袋护理方法。结论:膀胱癌行膀胱全切+输尿管腹壁双造口患者术后采用免造口袋法护理操作简单,患者皮肤舒适度高,费用低。Objective: To explore the comparison on effect of nursing care method for button-free ostomy bags and nursing care method for button-up ostomy bags in postoperative patients with bladder cancer undergoing radical cystectomy and ureter-abdomen double colostomy.Methods: During May 1,2018 to May 31,2021,76 postoperative patients with bladder cancer performing radical cystectomy and ureter-abdomen double colostomy were selected as the research objects, according to the selection by patients for postoperative nursing methods after 3 months were divided into the button-free bag group and the burron-up group each with 38 cases.3 months after operation, the adoption of the world health organization quality of life scale short form(WHOQOL-BREF)for assessment and comparison on two methods of nursing in patients′ quality of life.Three months after another nursing method was adopted, six months after patients had the freedom to choose what kind of nursing method eventually and record reason, and analysis was made on influencing factors.Results: The WHOQOL-BREF scores in patients who adopted the button-free bag nursing method after 3 months were higher than in those who adopted the button-up bag nursing method(P<0.05).Age, colostomy nursing methods, participation in related activities three months postoperatively were the independent factors for WHOQOL-BREF scores in patients(P<0.05).After 6 months, 54 patients selected button-free ostomy bag nursing method, 22 patients selected button-up ostomy bag nursing method.Conclusion: The nursing care method for button-free ostomy bags in postoperative patients with bladder cancer undergoing radical cystectomy and ureter-abdomen double colostomy is simple in operation, with high skin comfort in patients, and the cost is low.
