新生儿脐静脉导管相关性肝损伤的影像学表现

摘　　要：目的分析新生儿脐静脉导管(UVC)相关性肝损伤的影像学表现。方法回顾性分析行UVC置管术后影像学检查发现肝脏受损的10例患儿的置管相关临床情况及影像学表现。结果10例患儿中,X线平片显示4例肝影增大,1例腹部膨隆,5例肝区有气体影。CT检查10例肝区均见边界不清、形态不规则低密度病灶,无明显包膜;9例病灶内有气体;疑5例病灶内局部存在少许脂肪密度影;3例出现肝周或腹腔少量积液;1例病灶内有出血;CT平扫5例,1例患儿经UVC注入的对比剂渗入肝区及腹腔;CT平扫及增强扫描5例中,1例病灶周围无明显强化,其余4例病灶周围可见强化。结论新生儿UVC相关性肝损伤的影像学表现具有一定特征性,结合临床特点及UVC置管病史有助于做出诊断。Objective To analyze the imaging manifestations of the umbilical venous catheter(UVC)associated liver injury in neonates.Methods The clinical and imaging features of 10 children with liver injury after umbilical vein catheterization were retrospectively analyzed.Results In this group of 10 cases,plain X-ray films showed liver region enlargement in 4 cases,abdominal distention in 1 case,and liver had gas density in 5 cases.CT examination of 10 cases showed low density lesions with obscure boundary and irregular shapes in the liver,without obvious capsule.There was gas in the lesions of 9 cases.A little fat density was suspected in the lesions of 5 cases.A little perihepatic effusion or peritoneal effusion occurred in 3 cases.One case had hemorrhage within the lesion.Among the 5 cases with plain CT scans,1 case was injected with contrast media by UVC and the fluid infiltrated the liver and abdominal cavity.Among the 5 cases that received plain and enhanced CT scanning,1 case had no obvious enhancement around the lesions,while the other 4 cases had enhancement around the lesions.Conclusions The imaging features of umbilical venous catheter related liver injury in neonates have certain characteristics,and combining clinical features with the history of UVC is helpful to make a diagnosis.
