摘  要:目的探讨心外膜脂肪组织(EAT)相关参数测量值与冠状动脉病变之间的关系。方法采用CT对687例冠心病患者行冠状动脉CT血管成像(CCTA)检查,记录患者体质量指数(BMI),并利用后处理软件测量心外膜脂肪组织体积(EATV)、心周脂肪组织体积(PATV)、单支冠状动脉沟脂肪组织体积(SEATV)、左室游离壁厚度(LVWT)、右室游离壁厚度(RVWT)、冠状动脉钙化积分(CCS);对3支冠状动脉的冠状动脉狭窄程度(CS)及CCS进行测量。相关性用Spearman进行分析;采用Wilcoxon秩和检验进行组间数据比较,以P<0.05差异有统计学意义。结果EATV、PATV、LVWT及RVWT在冠心病组明显高于对照组,且与CS呈正相关;SEATV与相应CS及CCS均呈明显相关;按冠状动脉病变支数分组,多支病变组及双支病变组与单支病变组及对照组间存在显著性差异(P<0.05)。结论EATV、PATV、SEATV及心外膜脂肪厚度均与冠心病的发生呈正相关,可作为冠心病严重程度的有效预测指标,且SEATV可作为相应冠状动脉粥样硬化程度的独立的预测指标。Objective To investigate the correlation between the measured parameters of epicardial adipose tssue(EAT)and coronary artery disease.Methods687 patients who were diagnosed coronary heart disease underwent coronary computed tomography angiography(CCTA).The body mass index(BMI)of patients was recorded,as well as the epicardial adipose tissue volume(EATV),pericardial adipose tissue volume(PATV),adipose tissue volume of single coronary artery(SEATV),left ventricular free wall thickness(LVWT),right ventricular free wal thickness(RVWT),and coronary artery caleification score(CCS)were measured by post-processing software.In addition,the coronary artery stenosis(CS)and CCS of the three coronary arteries were also measured respectively.The correlation was analyzed by Spearman.The Wilcoron rank sum test was used to compare the data between groups,and the difference was statistically significant at P<0.05.Results EATV,PATV,LVWT and RVWT of coronary heart disease group were significantly higher than those in the control group,and were positively correlated with CS.In addition,SEATV was significantly correlated with CS and CCS.The data were divided into three groups according to the number of coronary artery lesions,and there were significant dfferences between the multi-vessel lesions group,the double-vessel lesions group and the single-vessel lesions group,the control group(P<0.05).Conclusion EATV,PATV,SEATV and the thickness of epicardial adipose tissue are positively correlated with the occurrence of coronary heart disease,and they can be used as effective predictors of the severity of coronary heart disease.The SEATV can also be used as an independent predictor of the degree of coronary atherosclerosis.
