摘  要:目的探讨3D梯度自旋回波(GRASE)序列磁共振胰胆管成像(MRCP)在胰胆管疾病影像诊断中的应用价值。方法收集96例胰胆管疾病患者的MRCP数据,比较2D快速自旋回波(TSE)序列与3D GRASE序列的图像质量和诊断结果。结果3D GRASE MRCP对胆总管、胆囊管及肝内胆管解剖结构的显示率优于2D TSE MRCP。2D TSE MRCP对胰管的显示优于3D GRASE MRCP,其中对胰管近段(Z=-0.831,P=0.406)、远段(Z=-1.396,P=0.163)的显示差异无统计学意义,对胰管中段的显示差异有统计学意义(Z=-2.084,P=0.037)。3D GRASE MRCP在胆道结石和胰胆管肿瘤的诊断上优于2D TSE MRCP。结论3D GRASE MRCP空间分辨率高,有助于胆道结石和肿瘤的明确诊断,与2D TSE MRCP相结合能更好地显示胰管。ObjectiveTo study the application value of 3D gradient and spin echo(GRASE)magnetic resonance cholangiopancreatography(MRCP)sequence in the imaging diagnosis of pancreaticobiliary duct diseases.MethodsThe MRCP data of 96 patients with pancreaticobiliary duct diseases were collected,the image quality and diagnosis results between 2D turbine spin echo(TSE)sequence and 3D GRASE sequence were compared.Results The anatomical structure display rate of choledoch,cystic duct and intrahepatic bile duct based on 3D GRASE MRCP was obviously better than that based on 2D TSE MRCP.2D TSE MRCP was superior to 3D GRASE MRCP in displaying the pancreatic duct.There was no significant difference in displaying the proximal(Z=-0.831,P=0.406)and distal(Z=-1.396,P=0.163)segments of the pancreatic duct between 2D TSE MRCP and 3D GRASE MRCP,but some significant differences in displaying the middle segment of the pancreatic duct(Z=-2.084,P=0.037).3D GRASE MRCP was more accurate than 2D TSE MRCP in the diagnosis of bile duct stones and pancreaticobiliary tumors.Conclusion 3D GRASE MRCP has higher spatial resolution,which is helpful for the diagnosis of bile duct stones and tumors,combine with 2D TSE MRCP is better for the display of pancreatic duct.
