摘　　要：目的:探讨5%碳酸氢钠注射液耳浴联合硝酸咪康唑乳膏治疗婴幼儿真菌性外耳道炎的临床效果。方法:选取63例真菌性外耳道炎患儿,按照随机数字法分为对照组30例和观察组33例。对照组采用硝酸咪康唑乳膏涂抹于外耳道内,观察组在对照组基础上加用5%碳酸氢钠注射液耳浴。观察并分析两组的外耳道炎真菌分布情况、临床疗效、真菌转阴率及不良反应情况。结果:63例共67耳真菌性外耳道炎患儿均可检测到真菌分布,主要构成为曲霉菌43株,占比为64.18%;观察组治疗总有效率93.94%高于对照组的83.33%(P<0.05);观察组真菌转阴率81.82%高于对照组的70.00%(P<0.05);两组均无明显不良反应。结论:5%碳酸氢钠注射液耳浴联合硝酸咪康唑乳膏治疗婴幼儿真菌性外耳道炎的效果较好,真菌转阴率高。Objective:To investigate the clinical efficacy of ear bath with 5% sodium bicarbonate injection combined with miconazole nitrate cream in the treatment of infantile with fungal external otitis.Methods:63 children with fungal external otitis were selected and randomized into two groups,with 30 cases in the control group and 33 cases in the observation group.Miconazole nitrate cream was applied to the external auditory canal of patients in the control group,while the observation group were treated with 5% sodium bicarbonate injection ear bath on the basis of the control group.The external auditory canal fungal distribution,clinical efficacy,fungal negative conversion rate and adverse reactions in patients were compared between both groups.Results:Fungal distribution could be detected in 67 ears of 63 children with fungal otitis externa,whose main composition was 43 aspergillus strains,accounting for 64.18%;after treatment,the total effective rate in the observation group was 93.94%,significantly higher than 83.33% in the control group(P<0.05);the fungal negative conversion rate of children in the observation group was 81.82%,significantly higher than 70.00% in the control group(P<0.05);and there was no obvious adverse reactions for both groups.Conclusion:5% sodium bicarbonate injection ear bath combined with miconazole nitrate cream has good clinical efficacy,with high fungal negative conversion rates.
