门诊药房多级多维用药交待体系的构建与实践

摘　　要：目的:提供一套方案以解决公立医疗机构用药交待工作中普遍存在的用药交待内容质量水平不一、缺乏同质化的问题。方法:以《浙江省用药交待规范》为依据,组建门诊药房用药交待工作实施项目小组,以患者需求及用药交待工作难点为导向,构建用药交待多级多维体系,建立适应当前诊疗环境的用药交待模式。结果:通过全新的用药交待体系,门诊药师用药交待率从40.6%提高到93.6%,门诊药房患者满意度从95.7%提高到99.7%。结论:多级多维用药交待体系能优化用药交待内容和培训考核机制,提升药师药学专业服务能力,保证用药交待的质量,促进用药交待服务的标准化。OBJECTIVE To provide a set of solutions to solve the problems of inconsistent content quality and lack of homogeneity in medication-explanation in public medical institutions.METHODS Based on the Standardization of Medication Interpretation in Zhejiang Province, a project group for the implementation of medication explanation in outpatient pharmacies was set up. Based on the needs of patients and the difficulties in medication explanation, a multi-level and multi-dimensional system for medication explanation was constructed, and a medication explanation model adapted to the current diagnosis and treatment environment was established.RESULTS Through the brand-new medication explanation system, the medication explanation rate of outpatient pharmacists increased from 40.6% to 93.6%, and the patient satisfaction rate of outpatient pharmacies increased from 95.7% to 99.7%.CONCLUSION The multi-level and multi-dimensional system for medication explanation optimizes the content of medication explanation and training and assessment mechanism, improves the pharmaceutical professional service ability of pharmacists, ensures the quality of medication explanation, and promotes the standardization of medication explanation service.
