AKA、CCP、血清RF联合检测在类风湿关节炎中的诊断价值

摘　　要：目的:探究抗角质蛋白抗体(AKA)、抗环瓜氨酸肽抗体(CCP)、血清类风湿因子(RF)联合检测对类风湿关节炎(RA)的诊断价值。方法:回顾性分析、收集2018年6月-2021年12月抚州市中医院接收的45例确诊为RA患者病历资料,另收集同期抚州市中医院接收的45例确诊为非RA自身免疫疾病患者病历资料,通过查阅资料,患者入院时均在抚州市中医院接受AKA、CCP、血清RF检测,分析AKA、CCP、血清RF联合检测对RA的诊断价值。结果:AKA、CCP、血清RF联合检测诊断RA的敏感度、准确度均高于三者单独检测,差异均有统计学意义(P<0.05);AKA、CCP、血清RF联合检测诊断RA的特异度、阳性预测值及阴性预测值与三者单独检测比,差异均无统计学意义(P>0.05)。AKA、CCP、血清RF联合检测诊断RA与临床综合诊断具有极好的一致性(Kappa=0.822),而AKA、CCP、血清RF单独检测诊断RA与临床综合诊断具有理想的一致性(Kappa=0.622、0.533、0.733)。结论:AKA、CCP、血清RF联合检测诊断RA的敏感度及准确度较高,且与临床确诊结果一致性好。Objective:To explore the diagnostic value of anti-keratin antibody (AKA),anti-cyclic citrulline peptide antibody (CCP) and serum rheumatoid factor (RF) in rheumatoid arthritis (RA).Method:Retrospective analysis was conducted to collect medical records of 45 patients diagnosed with RA Fuzhou Hospital of Traditional Chinese Medicine from June 2018 to December 2021,and 45 patients diagnosed with non-RA autoimmune diseases received from the Fuzhou Hospital of Traditional Chinese Medicine during the same period.According to the data,all patients received AKA,CCP and serum RF tests in Fuzhou Hospital of Traditional Chinese Medicine upon admission.The combined diagnostic value of AKA,CCP and serum RF in RA was analyzed.Result:The sensitivity and accuracy of the combined detection of AKA,CCP and serum RF were higher than those of the single detection of AKA,CCP and serum RF,the differences were statistically significant (P<0.05).There were no significant differences in specificity,positive predictive value and negative predictive value between combined detection of AKA,CCP and serum RF and the single detection of AKA,CCP and serum RF (P>0.05).The combined detection of AKA,CCP and serum RF showed excellent consistency with clinical comprehensive diagnosis (Kappa=0.822),while the single detection of AKA,CCP and serum RF showed ideal consistency with clinical comprehensive diagnosis (Kappa=0.622,0.533,0.733).Conclusion:The combined detection of AKA,CCP and serum RF has high sensitivity and accuracy in the diagnosis of RA,and is consistent with the clinical diagnosis.
