手术室整体护理在配合防旋型股骨近端髓内针内固定治疗老年股骨转子间骨折患者中的应用价值

摘　　要：目的探讨手术室整体护理在配合防旋型股骨近端髓内针(PFNA)内固定治疗老年股骨转子间骨折患者中的应用价值。方法选取莆田市第一医院于2019年1月至2021年1月收治的68例老年股骨转子间骨折患者作为研究对象,根据不同的护理方式分为对照组(34例)和观察组(34例)。对照组采用手术室常规护理模式,观察组采用手术室整体护理模式,对比两组的手术时间、住院时间、术后负面情绪、生活质量、护理服务满意度以及术后并发症发生情况。结果通过对比,观察组患者手术时间、住院时间均短于对照组(P<0.05);观察组患者躯体功能、心理功能、社会活动、物质生活评分均明显高于对照组(P<0.05);观察组患者总体满意度为100.00%,明显高于对照组的67.64%(P<0.05);经过护理干预,观察组患者的汉密尔顿抑郁量表(HAMD)和汉密尔顿焦虑量表(HAMA)评分均显著低于对照组(P<0.05);观察组术后并发症发生率为11.76%,明显低于对照组的47.06%(P<0.05)。结论手术室整体护理能够明显缩短PFNA内固定术的手术时间、患者住院时间,有效改善患者的负面情绪,使患者术后生活质量得到提升,得到患者的认可,同时可减少术后并发症的发生。Objective To explore the application value of holistic nursing in the operating room in the treatment of elderly patients with femoral intertrochanteric fractures with proximal femoral nail anti-rotation(PFNA)internal fixation.Methods A total of 68 elderly patients with femoral intertrochanteric fractures who were admitted to our hospital from January 2019 to January 2021 were selected as the research subjects,andthey were divided into the control group(34 cases)and the observation group(34 cases)using different nursing methods.example).The control group adopted the operating room routine nursing model,and the observation group adopted the operating room overall nursing model.The operation time,hospital stay,postoperative negative emotions,quality of life,nursing service satisfaction and postoperative complications were compared between the two groups.Results By comparison,the operation time and hospitalization time of the observation group were shorter than those of the control group(P<0.05).The scores of physical function,psychological function,social activity and material life were significantly higher than those of the control group(P<0.05).The overall satisfaction rate of observation group was 100.00%,which was significantly higher than that of the control group(67.64%)(P<0.05).After nursing in advance,the levels of HAMA and HAMD score were significantly lower than those in the control group(P<0.05).The postoperative complication rate in this group was 11.76%,significantly lower than that in the control group(47.06%)(P<0.05).Conclusion Overall nursing in the operating room PFNA internal fixation treatment can significantly shorten the operation time and hospitalization time,effectively improve the negative emotions of patients,improve the quality of life of patients after surgery,be recognized by patients,and reduce postoperative complications.It has a very significant clinical effect.
