拉西地平、西拉普利联合治疗原发性高血压的效果观察

摘　　要：目的:观察拉西地平联合西拉普利治疗原发性高血压的临床效果及左室重构的影响。方法:选取三明市第一医院2019年1月-2020年12月接收的54例原发性高血压患者,根据双盲法将其分为研究组与对照组,各27例,其中对照组给予拉西地平治疗,研究组在以上基础给予西拉普利,两组均持续治疗6周。观察各组血压[收缩压(SBP)、舒张压(DBP)]、左室重构指标[左房内径(LAD)、左室射血分数(LVEF)、左室重量指数(LVMI)]的变化情况,并对其临床疗效及安全性进行客观性评价。结果:治疗前两组SBP、DBP、LAD、LVEF、LVMI比较,差异均无统计学意义(P>0.05),治疗6周后两组SBP、DBP、LAD、LVMI均较治疗前降低,LVEF均较治疗前增高(P<0.05),且研究组SBP、DBP、LAD、LVMI均低于对照组,LVEF高于对照组(P<0.05);研究组总有效率明显高于对照组(P<0.05),研究组不良反应发生率略高于对照组,但差异无统计学意义(P>0.05)。结论:拉西地平联合西拉普利治疗原发性高血压,不仅具有较好的降压效果,还能改善患者左室重构,疗效及安全性良好,值得推广。Objective:To observe the clinical effect of Lacidipine combined with Cilazapril in the treatment of essential hypertension and the influence on left ventricular remodeling.Method:A total of 54 patients with essential hypertension admitted to the First Hospital of Sanming City from January 2019 and December 2020 were selected.They were divided into study group and control group by double-blind method with 27 cases in each group.The control group was treated with Lacidipine,and the study group was treated with Cilazapril on this basis.All patients were given 6 weeks of continuous treatment.The changes in blood pressure[systolic blood pressure(SBP),diastolic blood pressure(DBP)]and left ventricular remodeling indexes[left atrial diameter(LAD),left ventricular ejection fraction(LVEF),left ventricular mass index(LVMI)]were observed.Clinical effect and safety were evaluated.Result:There were no statistically significant differences in SBP,DBP,LAD,LVEF or LVMI between the two groups before treatment(P>0.05).After 6 weeks of treatment,SBP,DBP,LAD and LVMI were decreased,and LVEF was increased in the two groups(P<0.05).SBP,DBP,LAD and LVMI in the study group were lower than those in the control group,and LVEF was higher than that in the control group(P<0.05).The total effect rate of study group was significantly higher than that of control group(P<0.05),and incidence of adverse reactions of study group was slightly higher than that of control group,but there was no significant difference(P>0.05).Conclusion:Lacidipine combined with Cilazapril can not only well lower blood pressure but also improve left ventricular remodeling in patients with essential hypertension,with good curative effect and safety.It is worth promoting.
