针刀联合牵伸法对腓肠肌挛缩大鼠步态和肌肉收缩性能影响研究

摘  要:目的观察针刀联合牵伸法对腓肠肌挛缩大鼠踝关节背屈关节活动度(ROM)、步态、肌肉收缩性能的影响,探讨针刀联合牵伸法对肌肉挛缩的治疗效果。方法45只Wistar雄性大鼠随机分为空白组、模型组、牵伸组、针刀+牵伸组及针刀+牵伸远期组(远期组),每组9只,采用将右后肢踝关节跖屈位固定4周的方法复制腓肠肌挛缩模型。造模成功后牵伸组在大鼠右踝关节背屈方向垂直于足底区施加3 N的力拉伸腓肠肌,10次/d,30 s/次,间隔30 s/次,共治疗10 d;针刀+牵伸组于大鼠右侧腓肠肌肌腹中部进行针刀治疗1次后,再行与牵伸组相同的治疗10 d;远期组治疗同针刀+牵伸组,治疗10 d后再自由活动3周。各组大鼠于干预后测量大鼠ROM,应用DigiGaitTM步态分析、BL-420S生物机能系统采集肌肉收缩力学性能变化。结果①ROM:针刀+牵伸组ROM较模型组、牵伸组明显恢复(P<0.001,P<0.01)。远期组ROM较针刀+牵伸组明显恢复(P<0.001)。②步态分析:针刀+牵伸组足印面积、足印长度较模型组改善明显(P<0.001,P<0.001),较牵伸组改善明显(P<0.01,P<0.05)。远期组足印面积、足印长度较针刀+牵伸组改善明显(P<0.05,P<0.05)。而在步幅、摆动相、支撑相上牵伸组与针刀+牵伸组比较差异无统计学意义(P>0.05)。③骨骼肌力学收缩性能:针刀+牵伸组腓肠肌最大单收缩幅度较模型组、牵伸组明显改善(P<0.001;P<0.05),同时腓肠肌最大强直收缩幅度也明显改善(P<0.001,P<0.05)。远期组腓肠肌最大单收缩幅度及最大强直收缩幅度较针刀+牵伸组均明显改善(P<0.05,P<0.05)。④肌肉湿重:针刀+牵伸组肌肉湿重较模型组、牵伸组明显增加(P<0.001,P<0.05)。远期组肌肉湿重较针刀+牵伸组明显增加(P<0.001)。肌肉长度:针刀+牵伸组肌肉长度较模型组增加明显(P<0.01);而与牵伸组比较差异无统计学意义(P>0.05)。远期组较针刀+牵伸组肌肉长度增加明显(P<0.001)。�Objective To observe the effects of acupotomy combined with passive stretching on the range of motion of ankle joint dorsiflexion(ROM),gait and muscle contractile characteristics in gastrocnemius contracture rats,and to explore the therapeutic effect of acupotomy combined with passive stretching on muscle contracture.Methods Forty-five male Wistar rats were randomly divided into 5 groups:control group(CON,n=9),model group(MOD,n=9),passive stretching group(S,n=9),acupotomy+passive stretching group(A+S,n=9)and acupotomy+passive stretching long term group(A+S+L,n=9).The gastrocnemius contracture model was established by the right hindlimb immobilization in full plantar flexion using the immobilization device for 4 weeks.After successful modeling,the passive stretching group(S)was treated with 3 N force stretching the contracture gastrocnemius of the rats'right hindlimb for 30 s each at 30 s intervals,a total of 10 days;In the acupotomy+passive stretching group(A+S)was treated with acupotomy once in total,and then the same stretching method was used for 10 days.The acupotomy+passive stretching long-term group(A+S+L)was treated with the same method as the A+S group,then after 10 days of treatment,samples were collected after 3 weeks of free activity.After treatment,the ROM,DigiGaitTM analysis and the gastrocnemius contractile characteristics were measured.Results①ROM:compared to those in the MOD and S groups,the ROM was significantly increased in A+S group(P<0.001 and P<0.01).Compared to those in the A+S groups,the ROM was significantly increased in A+S+L group(P<0.001).②Gait analysis:compared to those in the MOD group,the paw area and paw length were significantly improved in A+S group(P<0.001 and P<0.001).Compared to those in the S group,the paw area and paw length were significantly improved in A+S group(P<0.01 and P<0.05).Compared to those in the A+S groups,the paw area and paw length were significantly improved in A+S+L group(P<0.05 and P<0.05).There were no significant differences in stride length,swing and
