益气活血方联合抗生素治疗慢性阻塞性肺疾病急性发作临床疗效及对患者纤维蛋白原的影响

摘　　要：目的观察益气活血方联合抗生素对慢性阻塞性肺疾病(COPD)急性发作患者临床疗效及纤维蛋白原影响。方法将我院就诊的COPD急性发作患者80例随机分为对照组(仅用抗生素等基础疗法治疗)与观察组(益气活血方联合抗生素等基础疗法治疗)各40例。比较两组患者疗效,治疗前后动脉血氧分压(PaO_(2))、动脉二氧化碳分压(PaCO_(2))、纤维蛋白原(FIB)、D-二聚体(D-D)、第1秒用力呼气量占预计值百分比(FEV_(1))、FEV_(1)/用力肺活量(FVC)。结果治疗后,观察组患者总有效率显著高于对照组(P<0.05);两组患者PaO_(2)、FEV_(1)、FEV_(1)/FVC增高,PaCO_(2)、FIB、D-D下降,观察组患者PaO_(2)、FEV_(1)、FEV_(1)/FVC高于对照组,PaCO_(2)、FIB、D-D低于对照组(P<0.05)。结论益气活血方联合抗生素对COPD急性发作疗效确切,改善血气分析指标和血栓前状态,提高肺功能。Objective To observe the efficacy of Yiqi Huoxue Decoction combined with antibiotics on patients with acute exacerbation of chronic obstructive pulmonary disease(COPD)and its effect on fibrinogen.Methods Eighty patients with acute exacerbation of COPD were randomly divided into control group(treated with antibiotics and other basic therapies)and observation group(Yiqi Huoxue Decoction combined with antibiotics and other basic therapies),each 40 cases.The curative effects of the two groups were compared.Arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2)),fibrinogen(FIB),D-dimer(D-D),and Fored expiratory volume in the first second(FEV_(1)),FEV_(1)/forced vital capacity(FVC)were compared before and after treatment.Results After treatment,the total effective rate was higher in observation group than in control group(P<0.05);PaO_(2),FEV_(1),FEV_(1)/FVC increased in both groups,PaCO_(2),FIB,D-D decreased in both groups,PaO_(2),FEV_(1),FEV_(1)/FVC were higher in observation group than in control group,PaCO_(2),FIB,D-D were lower than in control group(P<0.05).Conclusion Yiqi Huoxue Decoction combined with antibiotics has a definite curative effect on acute exacerbation of COPD,which can improve blood gas analysis indexes and prethrombotic state,and improve pulmonary function.
