摘  要:目的观察六君子汤联合全程营养管理对食管癌同步放化疗毒副作用、营养及免疫状态的影响。方法将我院收治的食管癌同步放化疗患者120例按照单双号法为对照组(59例,予以全程营养管理)与观察组(61例,对照组基础联合六君子汤治疗)。连续治疗2个月,比较两组的临床疗效、中医证候积分;两组患者提供的主管整体营养状况评定量表(PG-SGA)得分、血红蛋白(Hb)、血清白蛋白(Alb)、体质量指数(BMI)、免疫球蛋白A(IgA)、免疫球蛋白G(IgG)和免疫细胞因子CD3^(+)和CD4^(+)水平比较营养和免疫状态,比较两组放化疗毒副作用情况。结果治疗后,观察组临床总有效率高于对照组,且证候积分低于对照组(P<0.05);观察组Hb、Alb、BMI高于对照组,且PG-SGA低于对照组(P<0.05);观察组IgA、IgG、CD3^(+)和CD4^(+)水平均高于对照组(P<0.05),观察组放化疗毒副作用总发生率低于对照组(P<0.05)。结论六君子汤联合全程营养管理疗效明显,可改善患者的营养和免疫状态,有助于减轻患者放化疗毒副反应。Objective To observe the effects of Liujunzi Decoction combined with whole-course nutrition management on the toxic and side effects of concurrent chemoradiotherapy,nutritional status and immune status of patients with esophageal cancer.Methods A total of 120 patients with esophageal cancer undergoing concurrent chemoradiotherapy were divided into control group(n=59,given whole-course nutrition management)and observation group(n=61,combined with Liujunzi Decoction on the basis of control group)according to the odd-even number method.After 2 months of continuous treatment,the clinical efficacy and scores of TCM syndromes of the two groups were compared.The Patient-Generated Subjective Global Assessment(PG-SGA)score and hemoglobin(Hb),serum albumin(Alb)and body mass index(BMI)and levels of immunoglobulin A(IgA),immunoglobulin G(IgG)and immunocytokines CD3^(+)and CD4^(+)were used to compare the nutritional status and immune status.The number of patients with toxic and side effects of chemoradiotherapy in the two groups was compared.Results After treatment,the total clinical effective rate was higher in observation group than in control group,and syndrome scores were lower than in control group(P<0.05);the Hb,Alb and BMI in observation group were higher than in control group while the PGSGA score was lower than in control group;the IgA,IgG,CD3^(+)and CD4^(+)levels in observation group were higher than in control group(P<0.05);the total incidence rate of toxic and side effects inchemoradiotherapy in observation group was lower than in control group(P<0.05).Conclusion Liujunzi Decoction combined with whole-course nutrition management has an obvious efficacy.It can improve the nutritional status and immune status,and help relieve the toxic and side effects of chemoradiotherapyof patients.
