摘  要:目的探讨社区获得性肺炎(CAP)新生儿外周血维生素A(VA)水平变化及其与中性粒细胞功能的关系。方法选取2019年11月至2021年11月在该院住院的62例CAP新生儿(CAP组)作为研究对象,并根据不同病原体感染分为病毒感染组、细菌感染组、病毒与细菌混合感染组(混合感染组);选取同期在该院体检健康的30例新生儿作为对照组。检测和比较各组新生儿外周血VA水平、中性粒细胞吞噬率及吞噬指数,并分析CAP新生儿VA水平与中性粒细胞吞噬率及吞噬指数的关系。结果CAP组VA水平、中性粒细胞吞噬率及吞噬指数均明显低于对照组,差异均有统计学意义(P<0.05);而病毒感染组、细菌感染组、混合感染组之间VA水平、中性粒细胞吞噬率及吞噬指数的差异均无统计学意义(P>0.05)。CAP组临床型VA缺乏及亚临床型VA缺乏发生率明显高于对照组(P<0.05),对照组可疑亚临床型VA缺乏发生率高于CAP组(P<0.05)。CAP组中VA<0.70μmol/L患儿的中性粒细胞吞噬率及吞噬指数均明显低于VA≥0.70μmol/L患儿,差异有统计学意义(P<0.05)。结论CAP新生儿外周血VA水平、中性粒细胞吞噬率及吞噬指数下降,不同病原体感染所致的CAP新生儿上述指标无明显差异,CAP新生儿中性粒细胞功能降低可能与VA水平低有关。Objective To investigate the change of peripheral blood vitamin A(VA)levels and its relationship with neutrophil function in neonates with community acquired pneumonia(CAP).Methods Sixty-two hospitalized newborns with CAP(CAP group)in this hospital from November 2019 to November 2021 were selected as the study subjects and divided into the viral infection group,bacterial infection group,and virus-bacterial mixed infection group(mixed infection group)according to different pathogens infection.Thirty neonates undergoing health checkup during the same period were selected as the control group.The peripheral blood VA level,neutrophil phagocytic rate and phagocytic index were detected and the results were compared among the groups,and the relationship between the VA level with neutrophil phagocytic rate and phagocytic index in CAP neonates was analyzed.Results The VA level,neutrophil phagocytic rate and phagocytic index in the CAP group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05),but which had no statistically significant differences among the viral infection group,bacterial infection group and mixed infection group(P>0.05).The incidence rates of clinical and subclinical VA deficiency in the CAP group were significantly higher than those in the control group(P<0.05),and the incidence rate of suspected subclinical type VA deficiency in the control group was higher than that in the CAP group(P<0.05).In CAP group,neutrophil phagocytic rate and phagocytic index of the neonates with VA level<0.70μmol/L were significantly lower than those with VA level≥0.70μmol/L,and the difference was statistically significant(P<0.05).Conclusion Peripheral blood VA level,neutrophil phagocytic rate and phagocytic index in the neonatal patients with CAP are decreased,the above indicators in neonates with CAP caused by different pathogens infection have no obvious differences.The decrease of neutrophils function in neonates with CAP may be related to the low VA level.
