内外两侧切口双钢板内固定术治疗胫骨平台骨折的疗效

摘　　要：目的分析膝关节内外两侧切口双钢板内固定术对复杂胫骨平台骨折的临床疗效。方法选取我院收治的复杂胫骨平台骨折行手术治疗的患者98例,随机分为两组,每组49例。对照组采用膝关节前正中纵切口双钢板内固定术,观察组采用膝关节内外两侧切口双钢板内固定术,比较两组的临床疗效。结果观察组住院天数少于对照组,骨折愈合时间及患肢负重开始时间均短于对照组(P<0.05);两组术中出血量比较,P>0.05;观察组手术时间长于对照组(P<0.05);术后12个月观察组胫骨平台后倾角、内翻角均小于对照组(P<0.05);观察组术后6、12个月膝关节Rasmussen评分高于对照组(P<0.05)。结论膝关节内外两侧切口双钢板内固定术治疗复杂型胫骨平台骨折能缩短住院天数,有利于骨折愈合,使患者膝关节功能最大程度恢复,值得临床推广。Objective To analyze the clinical effect of double plate internal fixation with internal and external incisions in the treatment of fracture of tibial plateau.Methods 98 patients with complex tibial plateau fracture who underwent surgical treatment were selected and randomly divided into the control group and the observation group,49 cases in each group.The control group were treated with double plate internal fixation with anterior midline incision of knee joint,and the observation group were treated with double plate internal fixation with internal and external incisions.The clinical efficacy of the two groups was compared.Results The hospitalization time,healing time of fracture and weight bearing time of affected limb in the observation group was shorter than those in the observation group(P<0.05).There was no significant difference in intraoperative blood loss between the observation group and the control group(P>0.05).The operation time of the observation group was longer than that of the control group(P<0.05).12 months after operation,the caster angle and varus angle of tibial plateau in the observation group were smaller than those in the control group(P<0.05).The Rasmussen score of knee joint in the observation group was higher than that in the control group 6 months and 12 months after surgery(P<0.05).Conclusion The treatment of tibial plateau fracture by medial-lateral approach combined with double internal fixation can shorten the length of hospital stay,accelerate fracture healing,restore the original anatomical structure,and maximize the recovery of knee function,for reference.But in the clinical work,we should also consider the fracture of patients reasonable selection.
