改良针状电刀切除会厌囊肿的疗效

摘　　要：目的 比较改良针状电刀和传统高频电刀切除会厌囊肿的临床疗效。方法 102例行会厌囊肿切除术的患者随机均分为两组,针状电刀组采用显微镜支撑喉镜下改良针状电刀,高频电刀组采用显微镜支撑喉镜下传统A5高频电刀。比较两组手术时间、术中出血量和术后咽部VAS疼痛评分、住院时间、伪膜脱落时间、复发率及并发症发生率。结果 与高频电刀组相比,针状电刀组手术时间和术后住院时间短、术中出血量少且术后咽部VAS疼痛轻、伪膜脱落时间短、复发率和并发症发生率低(P<0.05)。结论 与传统高频电刀相比,改良针状电刀切除会厌囊肿具有手术时间短、术中出血少、术后疼痛轻、术后并发症发生率和复发率低的优点。Objective To compare the clinical efficacy of epiglottic cyst resection with modified needle-shaped electric knife and with conventional high-frequency electric knife.Methods A total of 102 patients with epiglottic cyst were randomly and equally divided into two groups.The epiglottic cyst was removed by modified needle-shaped electric knife(group A) or conventional high-frequency electric knife(group B) through lateral open laryngoscope under microscope.The operation time, intraoperative blood loss, postoperative pharyngeal VAS pain score, postoperative hospital stay, postoperative pseudomembranous shedding time, recurrence rate and the incidence of complications were compared between the two groups.Results Compared with group B,group A had shorter operation time and postoperative hospital stay, less intraoperative blood loss, shorter postoperative pseudomembranous shedding time, and lower postoperative pharyngeal VAS pain score, lower incidence rates of recurrence and complications(P<0.05).Conclusion Compared with conventional high-frequency electric knife, modified needle-shaped electric knife has the advantages of shorter operation time, less intraoperative blood loss and postoperative pain, and lower incidence of recurrence and complications in the excision of epiglottic cyst.
