脊髓横断后艾灸足三里对胃黏膜损伤大鼠Nrf2/HO-1信号通路的影响

摘　　要：目的:观察艾灸足三里通过调节Nrf2/HO-1信号通路对应激性胃黏膜损伤大鼠的治疗作用,并探讨其胃黏膜保护作用与脊髓的关系。方法:SPF级SD大鼠50只,按体质量随机分为5组:空白组、模型组、药物组、艾灸组及手术+艾灸组。采用无水乙醇结合阿司匹林混悬液灌胃建立大鼠胃黏膜损伤模型,艾灸组艾灸大鼠双侧“足三里”穴;药物组给予奥美拉唑肠溶片溶液(0.2 mg/kg)灌胃,连续治疗8 d;手术+艾灸组予以脊髓横断手术,术后进行胃黏膜损伤造模及艾灸治疗。治疗结束后,应用Guth法计算大鼠胃黏膜损伤指数(UI),HE染色观察大鼠胃黏膜形态学变化,ELISA实验检测血清中超氧化物歧化酶(SOD)、丙二醛(MDA)、一氧化氮(NO)和谷胱甘肽过氧化物酶(GSH-Px)水平的变化。Western-blot和RT-PCR检测胃黏膜组织中一氧化氮合酶(iNOS)、环氧合酶-2(COX-2)、核转录因子E2相关因子2(Nrf2)和血红素加氧酶-1(HO-1)的蛋白及mRNA表达。结果:与模型组相比,艾灸组大鼠UI值显著降低,差异具有统计学意义(P<0.01);胃黏膜组织损伤明显改善,明显降低了血清中MDA、NO水平,升高了SOD、GSH-Px水平,差异具有统计学意义(P<0.01);显著下调了胃黏膜组织中iNOS和COX-2的蛋白及mRNA表达水平,上调了Nrf2和HO-1的表达水平,差异具有统计学意义(P<0.01)。手术+艾灸组大鼠UI值、血清中MDA、NO水平及胃黏膜组织中iNOS和COX-2的表达较艾灸组升高,血清SOD、GSH-Px的水平及黏膜组织Nrf2和HO-1的表达水平明显降低,差异具有统计学意义(P<0.05)。结论:艾灸足三里可通过Nrf2/HO-1信号通路有效改善大鼠应激性胃黏膜损伤,脊髓参与了其修复作用及信号传导的过程。Objective:To observe the therapeutic effect of moxibustion at Zusanli(ST36)on stress-induced gastric mucosal damage in rats by regulating Nrf2/HO-1 signaling pathway,and to explore the relationship between the protective effect of gastric mucosa and spinal cord.Methods:50 SPF SD rats were randomly divided into the blank group,the model group,the medication group,the moxibustion group and the operation plus moxibustion group according to body weight,with 10 rats in each group.The model of stress-induced gastric mucosal damage was prepared by gavage of ethanol and Aspirin suspension.The moxibustion group was treated with moxibustion at bilateral ST36,the medication group was treated with Omeprazole enteric coated tablet solution(0.2 mg/kg)by gavage for 8 consecutive days,and the operation plus moxibustion group was given spinal cord transection and muscosal injury modeling and moxibustion after the operation.The gastric mucosal injury index(UI)was calculated by Guth method,the morphological changes of gastric mucosa were observed by HE staining,and the serum levels of superoxide dismutase(SOD),malondialdehyde(MDA),nitric oxide(NO)and glutathione peroxidase(GSH-Px)were detected by ELISA after the treatment.The protein and mRNA expressions of nitric oxide synthase(iNOS),cyclooxygenase-2(COX-2),nuclear factor E2 related factor 2(Nrf2)and heme oxygenase-1(HO-1)in gastric mucosa were detected by Western blot and RT-PCR.Results:Compared with those in the model group,the UI value was significantly decreased(P<0.01),the gastric mucosal tissue injury was significantly improved,the serum levels of MDA and NO were significantly decreased,and the levels of SOD and GSH-Px were increased(P<0.01),the protein and mRNA expression levels of iNOS and COX-2 were significantly down-regulated,and the expression levels of Nrf2 and HO-1 were up-regulated in the moxibustion group(P<0.01).Compared with those in the moxibustion group,the UI value,the serum levels of MDA and NO,the expressions of iNOS and COX-2 in gastric mucosa were increase
