摘 要:目的利用多焦视网膜电图(mfERG)与频域光学相干断层扫描(SD-OCT)分析非增生性糖尿病视网膜病变(NPDR)患者mfERG和黄斑厚度的改变及二者之间的关系。方法选取2020年11月至2021年11月在郑州大学第一附属医院眼科确诊为NPDR的40例患者(60只眼)作为观察组;同期选取正常人21例(31只眼)作为对照组。观察组参考早期治疗糖尿病视网膜病变研究(ETDRS)中糖尿病黄斑水肿的定义分为黄斑水肿组21例(34只眼)和无黄斑水肿组19例(26只眼)。行SD-OCT、mfERG检查,观察黄斑厚度和mfERG的改变及二者之间的关系。结果黄斑水肿组、无黄斑水肿组、对照组患者的黄斑中心区视网膜厚度(CST)分别为(324.29±48.57)、(261.00±18.61)、(249.87±13.58)μm。无黄斑水肿组和黄斑水肿组的CST厚于对照组(P<0.05),且黄斑水肿组CST厚于无黄斑水肿组(P<0.05)。与对照组相比,无黄斑水肿组的1、2、4、5环P1波潜伏期延长(P<0.05);与对照组相比,黄斑水肿组的1~5环P1波潜伏期延长(P<0.05);黄斑水肿组的1环P1波潜伏期长于无黄斑水肿组(P<0.05)。无黄斑水肿组的1、2、4环P1波振幅密度低于对照组(P<0.05),黄斑水肿组的1~5环P1波振幅密度低于对照组(P<0.05);黄斑水肿组1、2、3环P1波振幅密度低于无黄斑水肿组(P<0.05)。黄斑水肿组1环P1波振幅密度与CST呈负相关;1环P1波潜伏期与CST呈正相关。结论随着DR程度的进展、糖尿病黄斑水肿的出现,黄斑逐渐增厚,mfERG的潜伏期延长、振幅密度降低,CST与mfERG的潜伏期和振幅密度有相关性。Objective Using multifocal electroretinography(mfERG)and spectral-domain optical coherence tomography(SD-OCT)to analyze the changes of mfERG and macular thickness in patients with non-proliferative diabetic retinopathy(NPDR)and their relationship.Methods A total of 40 patients(60 eyes)who were diagnosed with NPDR in the Department of Ophthalmology of the First Affiliated Hospital of Zhengzhou University from November 2020 to November 2021 were selected as the observation group,and 21 normal people(31 eyes)were selected as the control group.According to the definition of diabetic macular edema in ETDRS,the observation group were divided into macular edema group with 21 patients(34 eyes)and non-macular edema group with 19 patients(26 eyes).SD-OCT and mfERG were performed to observe the changes of macular thickness,mfERG and the relationship between them.Results The central subfield thickness(CST)of patients in the macular edema group the non-macular edema group and the control group were(324.29±48.57),(261.00±18.61)and(249.87±13.58)μm,respectively.The CST in the non-macular edema group and the macular edema group was thicker than that in the control group(P<0.05),and the CST in the macular edema group was thicker than that in the non-macular edema group(P<0.05).Compared with the control group,the implicit time of P1 wave in the ring 1,2,4 and 5 in the non-macular edema group were prolonged(P<0.05).Compared with the control group,the implicit time of P1 wave in the ring 1-5 in the macular edema group were prolonged(P<0.05).The implicit time of P1 wave in the ring 1 in the macular edema group was longer than that in the non-macular edema group(P<0.05).The amplitude density of P1 wave in the ring 1,2 and 4 in the non-macular edema group was lower than that in the control group(P<0.05),and the amplitude density of the P1 wave in the ring 1-5 in the macular edema group was lower than that in the control group(P<0.05).The amplitude density of P1 wave in the ring 1,2,3 in the macular edema group was lower than that in
