摘　　要：目的探讨乙型肝炎表面抗原(HBsAg)低反应性乙肝患者乙型肝炎病毒e抗原(HBeAg)表达与乙型肝炎病毒(HBV)基因型、DNA载量关系,分析其对抗病毒治疗的指导意义。方法选取2019年4月至2021年10月驻马店市第一人民医院收治的152例HBsAg低反应性(HBsAg定量<1000 IU·L^(-1))乙肝患者,采用PCR-荧光探针法定量检测HBV基因分型、DNA载量,电化学发光法定量检测HBeAg、HBsAg,速率法检测丙氨酸氨基转移酶(ALT),并检测白蛋白(ALB)、甲胎蛋白(AFP)、胆碱酯酶(CHE)。分析HBV基因型与HBeAg表达、DNA载量、丙氨酸氨基转移酶(ALT)的关系。结果B基因型乙肝患者HBeAg转阴率较C基因型乙肝患者高(P<0.05);治疗3个月后B基因型HBsAg较C基因型低,HBeAg表达较C基因型高,差异有统计学意义(P<0.05);B基因型的HBsAg与HBeAg表达呈负相关(r=-0.486,P<0.05),HBsAg与DNA载量无相关性(P>0.05);C基因型的HBsAg与HBeAg表达呈负相关(r=-0.419,P<0.05),HBsAg与DNA载量无关(P>0.05)。结论HBsAg低反应性乙肝患者B基因型、C基因型HBsAg与HBeAg表达均呈负相关,与DNA载量相关性不明显,明确HBsAg与HBeAg表达的关系并监测,对临床诊断、治疗及预后有积极意义。Objective To explore the relationship between the expression of hepatitis B virus e antigen(HBeAg)and the genotype and DNA load of hepatitis B virus(HBV)in patients with hepatitis B surface antigen(HBsAg)hyporeactivity,and to analyze their antiviral therapy guiding significance.Methods A total of 152 patients with hepatitis B with low HBsAg reactivity(HBsAg quantitative<1000 IU·L^(-1))admitted to Zhumadian First People’s Hospital from April 2019 to October 2021 were selected.PCR-fluorescent probe method was used to quantitatively detect HBV genotype and DNA load.Quantitative detection of HBeAg and HBsAg by electrochemiluminescence method,detection of alanine aminotransferase(ALT)by rate method,and detection of albumin(ALB),alpha-fetoprotein(AFP)and cholinesterase(CHE).The relationship between HBV genotype and HBeAg expression,DNA load and alanine aminotransferase(ALT)were analyzed.Results The negative rate of HBeAg in patients with hepatitis B genotype B was higher than that in patients with hepatitis B genotype C(P<0.05).After 3 months of treatment,the HBsAg of B genotype was lower than that of C genotype,and the expression of HBeAg was higher than that of C genotype(P<0.05).The expression of HBsAg in B genotype was negatively correlated with HBeAg expression(r=-0.486,P<0.05),but there was no correlation between HBsAg and DNA load(P>0.05).HBsAg of C genotype was negatively correlated with HBeAg expression(r=-0.419,P<0.05),but there was no correlation between HBsAg and DNA load(P>0.05).Conclusion The B genotype and C genotype HBsAg and HBeAg expression are negatively correlated in HBsAg hyporesponsive hepatitis B patients,and the correlation with DNA load is not obvious.It is of positive significance for clinical diagnosis,treatment and prognosis to identify and monitor the relationship between HBsAg and HBeAg expression.
