结直肠癌患者腹腔镜术后发生动力性肠梗阻的影响因素

摘　　要：目的探讨结直肠癌患者腹腔镜术后发生动力性肠梗阻的影响因素。方法选取2020年11月至2021年5月柘城中医院接受腹腔镜下结直肠癌根治术的340例结直肠癌患者,以腹腔镜术后30 d动力性肠梗阻发生情况进行分组。由研究人员设计患者基线资料填写表,询问并记录研究所需资料,纳入可能的影响因素,采用logistic回归分析直肠癌患者腹腔镜术后发生动力性肠梗阻的影响因素。结果340例结直肠癌患者腹腔镜术后发生动力性肠梗阻45例,发生率为13.24%;发生组患者肿瘤部位、肿瘤病理分期、术前低蛋白血症占比及手术时间与未发生组患者对比,差异有统计学意义(P<0.05);组间其他资料对比,差异无统计学意义(P>0.05);logistic回归显示,结肠肿瘤、肿瘤病理分期Ⅲ~Ⅳ期、术前合并低蛋白血症、手术时间长是结直肠癌患者腹腔镜术后发生动力性肠梗阻的影响因素(OR>1,P<0.05)。结论结直肠癌患者腹腔镜术后发生动力性肠梗阻与结肠肿瘤、肿瘤病理分期为Ⅲ~Ⅳ期、术前合并低蛋白血症、手术时间长等因素有关。Objective To investigate the influencing factors of dynamic intestinal obstruction after laparoscopic surgery in patients with colorectal cancer.Methods A total of 340 patients with colorectal cancer undergoing laparoscopic radical resection of colorectal cancer in Zhecheng Hospital of Traditional Chinese Medicine from November 2020 to May 2021 were selected,and the incidence of dynamic intestinal obstruction 30 days after laparoscopic surgery was divided into groups.The researchers designed a form for patients'baseline data,inquired and recorded the data needed for the study,and included possible influencing factors.Logistic regression was used to analyze the influencing factors of dynamic intestinal obstruction in patients with rectal cancer after laparoscopic surgery.Results Among 340 patients with colorectal cancer after laparoscopic surgery,45 cases had dynamic intestinal obstruction,the incidence rate was 13.24%.Compared tumor location,tumor pathological stage,proportion of preoperative hypoproteinemia and operation time of patients in the occurrence group with those in the non occurrence group,the difference was statistically significant(P<0.05).There was no statistical significant difference in other data between groups(P>0.05).The results of logistic regression analysis showed that,colon tumor,tumor pathological stageⅢ-Ⅳ,preoperative hypoproteinemia and long operation time were the influencing factors of dynamic intestinal obstruction after laparoscopic surgery in patients with colorectal cancer(OR>1,P<0.05).Conclusion Dynamic intestinal obstruction after laparoscopic surgery in patients with colorectal cancer are related to colon tumor,tumor pathological stageⅢ-Ⅳ,preoperative hypoproteinemia,long operation time and so on.
