摘　　要：目的探究彩色多普勒超声在特殊部位早期异位妊娠诊断中的价值。方法回顾性分析2018年6月至2020年11月本院产科收治的81例疑似特殊部位异位妊娠并经手术治疗患者的超声影像学图像和临床资料,总结特殊部位妊娠的超声图像特点,比较经阴道彩色多普勒超声与经腹彩色多普勒超声及联合诊断对于特殊部位异位妊娠诊断的准确度、灵敏度、特异度,分析不同类型异位妊娠的准确度及孕囊相关检查(心管搏动、卵黄囊、附件包块、胚芽及盆腔积液)的检出结果。结果经阴道彩色多普勒超声检查诊断准确度高于经腹彩色多普勒超声(P<0.05),两种途径特异度、灵敏度比较差异无统计学意义;联合超声诊断准确度、灵敏度均高于经腹彩色多普勒超声、经阴道彩色多普勒超声检测,特异度高于经腹彩色多普勒超声(P<0.05);联合超声诊断与经阴道彩色多普勒超声特异度比较差异无统计学意义。经腹彩色多普勒超声对于输卵管间质妊娠检出率为71.43%,低于经阴道彩色多普勒超声的91.43%(P<0.05),其他部位检出率比较差异无统计学意义。经阴道彩色多普勒超声检查对于心管搏动、胚芽、卵黄囊、盆腔积液和附件包块的检出率均明显高于经腹彩色多普勒超声(P<0.05)。结论经腹联合经阴道彩色多普勒超声对特殊部位早期异位妊娠诊断的准确度、灵敏度较高,经阴道彩色多普勒超声对心管搏动、胚芽等微小变化检出率较高,在异位妊娠诊断中具有重要参考价值。Objective To explore the value of color Doppler ultrasound in the diagnosis of early ectopic pregnancy at special sites.Methods The ultrasound imaging images and clinical data of 81 patients with suspected ectopic pregnancy in special sites admitted to the Obstetrics Department of our hospital from June 2018 to November 2020 were retrospectively analyzed.The characteristics of ultrasound images of ectopic pregnancy in special sites were summarized.The accuracy,sensitivity and specificity of transvaginal and transabdominal color Doppler ultrasound and combined diagnosis in the diagnosis of ectopic pregnancy in special sites were compared.The accuracy of different types of ectopic pregnancy and the detection results of gestational sac related examinations(cardiac pulsation,yolk sac,accessory mass,germ and pelvic effusion)were analyzed.Results The diagnostic accuracy of transvaginal color Doppler ultrasound was higher than that of transabdominal color Doppler ultrasound(P<0.05).There was no significant difference in specificity and sensitivity between the two methods.The accuracy and sensitivity of combined ultrasound diagnosis were higher than those of transabdominal color Doppler ultrasound and transvaginal color Doppler ultrasound,and the specificity was higher than that of transabdominal color Doppler ultrasound(P<0.05).There was no significant difference in specificity between combined ultrasound diagnosis and transvaginal color Doppler ultrasound.The detection rate of transabdominal color Doppler ultrasound for tubal pregnancy was 71.43%,which was lower than 91.43%of transvaginal color Doppler ultrasound(P<0.05).There was no significant difference in the detection rate of other parts.The detection rate of cardiac tube beat,germ,yolk sac,pelvic effusion and adnexal mass in transvaginal color Doppler ultrasonography was significantly higher than that in transabdominal color Doppler ultrasonography(P<0.05).Conclusion Transabdominal combined transvaginal color Doppler ultrasonography has high accuracy and sensitiv
