布比卡因腰硬联合麻醉与硬膜外麻醉在老年胫骨远端骨折手术中的麻醉效果

摘　　要：目的探究布比卡因腰硬联合麻醉与硬膜外麻醉应用于老年胫骨远端骨折手术的麻醉效果。方法选取2019年5月至2021年10月于本院接受手术治疗的80例老年胫骨远端骨折患者作为研究对象,按照随机数字表法分为实验组与对照组,每组40例。对照组行布比卡因连续硬膜外麻醉(CEA),实验组行布比卡因腰硬联合麻醉(CSEA),比较两组麻醉药物用量、麻醉起效时间、麻醉效果。结果实验组麻醉药物用量明显少于对照组,麻醉起效时间明显短于对照组,差异有统计学意义(P<0.05)。实验组麻醉优良率为90.00%,明显高于对照组的72.5%,差异有统计学意义(P<0.05)。结论与布比卡因CEA麻醉方法相比,布比卡因CSEA应用于老年胫骨远端骨折手术患者能有效减少麻醉药物用量,缩短麻醉起效时间,麻醉效果良好,安全性高,值得临床推广应用。Objective To compare the anesthetic effect of bupivacaine combined spinal epidural anesthesia and epidural anesthesia in elderly patients with distal tibial fractures.Methods 80 elderly patients with distal tibial fractures in our hospital from May 2019 to October 2021 were selected as the research subjects,and they were divided into experimental group and control group according to the random number table method,with 40 cases in each group.The control group was given bupivacaine continuous epidural anesthesia(CEA),and the experimental group was given bupivacaine combined spinal epidural anesthesia(CSEA),the dosage of anesthetics,onset time of anesthesia,and the effect of anesthesia between the two groups were compared.Results The dosage of anesthetic in the experimental group was significantly less than that in the control group,and the onset time of anesthesia was significantly shorter than that in the control group,the difference was statistically significant(P<0.05).The excellent and good rate of anesthesia in the experimental group was 90.00%,which was significantly higher than 72.5%in the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with bupivacaine CEA anesthesia method,bupivacaine CSEA applied to elderly patients with distal tibial fracture surgery can effectively reduce the amount of anesthesia drugs,shorten the onset time of anesthesia,good anesthesia effect and high safety,and is worthy of clinical application.
