摘　　要：目的探讨耳内镜与显微镜联合上鼓室入路行中耳胆脂瘤切除的效果。方法抽取2016年1月至2021年1月濮阳市安阳地区医院收治的62例中耳胆脂瘤患者为研究对象,均予以耳内镜与显微镜联合上鼓室入路中耳胆脂瘤切除治疗,均随访1年以上。评价治疗效果,比较术前与术后3个月听力情况,记录随访1年的复发与感染情况。结果62例患者经手术治疗3个月后,鼓膜愈合60例,愈合率高达96.77%,另2例发生感染继发性穿孔,后经耳内镜联合显微镜手术后愈合。术后3个月,62例患者的气导听阈与气骨导差均低于术前,差异有统计学意义(P<0.05);随访1年时,复发1例,感染1例。结论中耳胆脂瘤采取耳内镜与显微镜联合上鼓室入路切除治疗,疗效较好,可显著改善听力指标,且复发率与感染率低。Objective To investigate the effect of resection of middle ear cholesteatoma via epitympanum under ear endoscope combined with microscope.Methods A total of 62 patients with middle ear cholesteamoma from January 2016 to January 2021 in the Anyang District Hospital of Puyang were selected as research subjects.They were all treated by resection of middle ear cholesteatoma via epitympanum under ear endoscopy combined with microscope.All patients were followed up for more than 1 year to evaluate the effect.The hearing condition before and 3 months after operation were compared,and the recurrence and infection were recorded during the 1-year follow-up.Results Among the 62 patients,the tympanic membrane healed in 60 cases after 3 months of surgical treatment,and the healing rate was as high as 96.77%.The remaining 2 cases suffered from infection and secondary perforation,which were healed by otoscopy combined with microscopy;3 months after operation,air conduction hearing threshold and air-bone conduction difference were significantly lower than those before operation,with significant difference(P<0.05).At 1-year follow-up,1 case had recurrence,and 1 case had infection.Conclusions Resection of middle ear cholesteatoma via epitympanum under ear endoscope combined with microscope not only has good curative effect,but also can significantly improve hearing indicators of patients,and has low recurrence rate and infection rate.
