摘 要:目的对比研究增强型体外反搏(Enhanced External Counterpulsation,EECP)期间产生的反搏波相比于正常脉动波对于狭窄血管血流动力学的影响机制,以此验证这种特殊的波形是否是EECP产生有效性的主要因素。方法本文建立了三种程度狭窄的血管模型,入口处分别采用反搏波与正常脉动波进行数值计算,研究反搏波对于流场、时均壁面剪切力(Time-averaged Wall Shear Stress,TAWSS)、振荡剪切系数(Oscillating Shear Index,OSI)以及相对停留时间(Relative Residence Time,RRT)的影响。结果在反搏波作用下,低TAWSS区域在30%狭窄血管减少比例为61.63%,而50%狭窄血管的减少比率为85.71%,70%狭窄血管减少了约4.08%。并且三种狭窄下的OSI均有所提高。值得注意的是RRT在70%狭窄血管下游增加了69.7%。结论对于狭窄程度小于50%的血管而言,反搏波具有积极的治疗作用,然而对于严重狭窄的血管,反搏波对于血流动力学的影响效果有所减弱。Objective Compared the counterpulsation wave generated during EECP to the normal pulsatile wave for stenotic blood vessels.The mechanism of hemodynamics was tested to verify whether this particular waveform is the main factor for the effectiveness of EECP generation.Methods In this paper,blood vessel models with three degrees of stenosis were established,and the counterpulsation wave and the normal pulsation wave were used for numerical calculation at the entrance.The effects of counterpulsation wave on flow field,Time-averaged Wall Shear Stress(TAWSS),Oscillating Shear Index(OSI)and Relative Residence Time(RRT)were studied.Results Under the action of counterpulsation wave,the reduction rate of 30%stenotic vessels in the low TAWSS region was 61.63%,while the reduction rate of 50%stenotic vessels was 85.71%,and the 70%stenotic vessels only decreased by 4.08%.And the OSI was improved in all three stenoses.Notably,RRT increased 69.7%downstream of 70%stenotic vessels.Conclusion For vessels with less than 50%stenosis,counterpulsation has a positive therapeutic effect,but for severely stenotic vessels,the effect of counterpulsation on hemodynamics is weakened.
