摘　　要：目的探讨游离带肌腱的尺动脉腕上穿支皮瓣修复在中小面积指背皮肤软组织复合伸指肌腱缺损患者中的应用效果。方法选取2020年1月至10月鹤山市人民医院收治的60例中小面积指背皮肤软组织复合伸指肌腱缺损患者作为研究对象,采用随机数字表法分为研究组与对照组,各30例。对照组采用复合皮瓣修复手术治疗,研究组采用游离带肌腱的尺动脉腕上掌侧穿支皮瓣修复手术治疗。比较两组患者的围手术期指标、术后12个月临床疗效及并发症。结果研究组的手术时间、首次下床时间、住院时间短于对照组,术中出血量少于对照组,术后12个月的疗效优良率高于对照组,术后12个月并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论游离带肌腱的尺动脉腕上穿支皮瓣修复手术操作简单且术后并发症少,可有效改善小面积指背皮肤软组织复合伸指肌腱缺损患者患指伸屈功能。Objective To investigate the effect of free supracarpal perforator flap of ulnar artery with tendon in the repair of small and medium-sized dorsal digital skin and soft tissue combined with extensor tendon defect.Methods A total of 60 patients with small and medium-sized dorsal digital skin and soft tissue combined with extensor tendon defect treated in People's Hospital of Heshan City from January to October 2020 were selected as the research objects.They were divided into the research group and the control group according to random number table method,with 30 cases in each group.The control group was repaired with composite flap,the research group was treated with free supracarpal perforator flap of ulnar artery with tendon.The perioperative indexes,clinical efficacy and complications 12 months after operation were compared between the two groups.Results The operative time,time to get out of bed for the first time and length of hospital stay in the research group were shorter than those in the control group.The intraoperative blood loss in the research group was less than that in the control group,the excellent and good efficacy rate 12 months after operation in the research group was higher than that in the control group,the total complication rate 12 months after operation in the research group was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion The repair of free supracarpal perforator flap of ulnar artery with tendon is simple and has few postoperative complications,which can effectively improve the extension and flexion function of fingers in patients with small dorsal digital skin and soft tissue combined with extensor tendon defect.
