摘 要:目的探讨机器人辅助下空心螺钉内固定治疗股骨颈骨折的临床疗效。方法回顾性分析2019~2021年因股骨颈骨折就诊于甘肃省人民医院并行空心螺钉的39例患者,其中传统方法组22例,机器人辅助组17例。两组患者的一般资料比较,差异无统计学意义。比较两组患者的手术时间、导针放置次数、一次性置钉成功率、术中透视次数、术中出血量、螺钉间的平行度以及Harris关节评分。结果机器人辅助组和传统方法组的操作时间分别为56.80±20.68min和72.73±25.10min,导针放置次数分别为3.06±0.24次和4.45±1.18次,术中透视次数分别为14.12±1.87次和21.32±4.08次,前后位螺钉的平行度分别为1.68°±0.44°和2.38°±0.43°,侧位螺钉的平行度分别为1.88°±0.51°和2.34°±0.37°,术中出血量分别为15.00±5.30ml和23.64±8.75ml,两组数据比较,差异有统计学意义。机器人辅助组和传统方法组的Harris评分分别为87.12±2.71分和85.91±4.12分,差异无统计学意义。结论和传统方法的手术比较,机器人辅助下空心螺钉内固定可以降低术中出血量、提高手术的精度并且可以降低术中因X线而造成的危害。Objective To investigate the clinical efficacy of robot-assisted hollow screw internal fixation for femoral neck fractures.Methods A total of 39 patients who visited Gansu Provincial People′s Hospital for femoral neck fractures with parallel hollow screws from 2019 to 2021 were retrospectively analyzed,including 22 cases in the traditional method group and 17 cases in the robot-assisted group.There was no statistical differences in the general information of the patients between the two groups.The operative time,number of guide pin placements,success rate of one-time nail placement,number of intraoperative fluoroscopy,intraoperative bleeding,parallelism between screws,and Harris joint score were compared between the two groups.Results The operation time was 56.80±20.68 min and 72.73±25.10 min for the robot-assisted group and the conventional method group,respectively.The number of guide needle placements was 3.06±0.24 and 4.45±1.18,respectively;the number of intraoperative fluoroscopic views was 14.12±1.87 and 21.32±4.08,respectively.The anterior-posterior 1.68°±0.44°and 2.38°±0.43°of parallelism of the lateral screws and 1.88°±0.51°and 2.34°±0.37°of parallelism of the lateral screws,respectively,and the intraoperative bleeding was 15.00±5.30 ml and 23.64±8.75 ml,respectively.The above data were statistically significant.Harris scores were 87.12±2.71 and 85.91±4.12 in the robot-assisted and conventional method groups,respectively,with no significant differences.Conclusion Robotic-assisted hollow screw fixation can reduce intraoperative bleeding,improve surgical precision,and reduce intraoperative X-ray hazards compared with traditional surgical methods.
