单孔电视胸腔镜手术治疗老年自发性气胸的效果分析

摘　　要：目的:分析单孔电视胸腔镜手术治疗老年自发性气胸的效果。方法:选择2016年1月-2020年12月南华大学附属长沙中心医院收治的88例老年自发性气胸患者作为研究对象,随机分为对照组与观察组,各44例。对照组实施采用传统胸腔镜手术治疗,观察组实施单孔胸腔镜手术治疗,比较两组手术指标、疼痛程度、气胸复发情况与并发症发生情况。结果:观察组术后第1、3、7天的主诉疼痛分级法评分均低于对照组,差异有统计学意义(P<0.05)。观察组手术总时间、术中出血量、术后引流量、引流管放置时间及住院时间均优于对照组,差异有统计学意义(P<0.05)。观察组气胸复发率、并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。结论:采取单孔电视胸腔镜手术治疗老年自发性气胸,能够获得更加显著的治疗效果,且此种手术操作简单,安全有效,手术后不会导致患者出现严重的疼痛感,值得推广。Objective:To analyze the efficacy of single-port video-assisted thoracoscopy in the treatment of spontaneous pneumothorax in the elderly.Methods:A total of 88 elderly patients with spontaneous pneumothorax who were treated in Changsha Central Hospital Affiliated to University of South China from January 2016 to December 2020 were selected as the study subjects and randomly divided into control group and observation group,with 44 cases in each group.The control group was treated with traditional thoracoscopic surgery,and the observation group was treated with single-port thoracoscopic surgery.The surgical indicators,pain degree,recurrence of pneumothorax and complications were compared between the two groups.Results:The pain grading scores by chief complaint of the observation group on 1,3,and 7 days after operation were lower than those of the control group,and the difference was statistically significant(P<0.05).The total operation time,intraoperative blood loss,postoperative drainage volume,drainage tube placement time and hospital stay of the observation group were better than those of the control group,and the differences were statistically significant(P<0.05).The recurrence rate of pneumothorax and the incidence of complications in the observation group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:For the elderly patients with spontaneous pneumothorax,single-port video-assisted thoracoscopic surgical treatment can obtain more significant therapeutic effect.This kind of operation is simple,safe and effective,and will not lead to too serious pain after operation.It is worthy of recommendation.
