非布司他联用四妙散加味治疗痛风性关节炎的临床疗效

摘　　要：目的观察非布司他联用四妙散加味治疗痛风性关节炎的临床疗效。方法回顾性选取2020年2月—2021年2月竹溪县中医院收治的痛风性关节炎患者78例,依据治疗方法分为单一组与联用组,各39例。单一组予以非布司他,联用组在单一组基础上予以四妙散加味,2组均治疗2个疗程。比较2组临床疗效,治疗前后症状评分、数字分级法(NRS)评分、关节功能评分、改良Barthel指数量表(MBI)评分、生存质量测定量表简表(QOL-BREF)评分、血尿酸(SUA)、C反应蛋白(CRP)、红细胞沉降率(ESR)、血肌酐(SCr)、p H值,痛风发作次数、完全缓解时间,不良反应发生情况。结果联用组治疗总有效率为94.87%,高于单一组的66.67%(χ^(2)=9.987,P=0.002)。治疗2个疗程后,联合组症状评分、NRS评分、关节功能评分低于单一组,MBI评分、QOL-BREF评分高于单一组(P<0.01)。治疗2个疗程后,联合组SUA、CRP、ESR、SCr水平低于单一组(P<0.05)。2组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论非布司他联用四妙散加味治疗痛风性关节炎的临床疗效确切,可有效改善临床症状,提高关节功能,减轻疼痛及炎症,且安全性较高。Objective To observe clinical efficacy of febuxostat combined with Simiao powder modified in the treatment of gouty arthritis.Methods A total of 78 cases of patients with gouty arthritis were selected from February 2020 to February 2021 in Zhuxi County Hospital of Traditional Chinese Medicine,which were divided into single group and combined group according to the treatment methods,with 39 cases in each group.The single group was given febuxostat alone,and the combined group was given Simiao powder modified on the basis of the single group.Both groups were treated for 2 courses.Clinical efficacy,symptom score,NRS score,joint function score,MBI score,QOL-BREF score,SUA,CRP,ESR,SCr,p H value before and after treatment,the number of gout attacks,complete remission time,the incidence of adverse reactions were compared between the two groups.Results The total effective rate of the combined group was 94.87%,was higher than66.67%of the single group(χ^(2)=9.987,P=0.002).After 2 courses of treatment,the symptom score,NRS score and joint function score of the combined group were lower than those of the single group,and the MBI score and QOL-BREF score were higher than those of the single group(P<0.01).After 2 courses of treatment,the levels of SUA,CRP,ESR and SCr in the combined group were lower than those in the single group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Febuxostat combined with Simiao powder modified have an exact clinical effect for gouty arthritis,which can effectively improve clinical symptoms,promote joint function,relieve pain and inflammation,and with high safety.
