复方杜仲健骨颗粒联合体外冲击波治疗早期股骨头坏死的临床疗效

摘　　要：目的观察复方杜仲健骨颗粒联合体外冲击波治疗早期股骨头坏死的临床疗效。方法选取2018年6月—2019年11月天津市公安医院收治的早期股骨头坏死患者84例,采用随机数字表法分为对照组和研究组,各42例。对照组采用体外冲击波治疗,研究组在对照组基础上采用复方杜仲健骨颗粒治疗,2组均连续治疗3个月。比较2组临床疗效,治疗前及治疗后3个月Harris髋关节评分(HHS)、日本骨科协会评估治疗(JOA评分)、中医症候积分,不良反应。结果研究组总有效率为90.48%,高于对照组的71.43%(χ^(2)=4.941,P=0.026)。治疗后3个月,2组HHS、JOA评分高于治疗前,且研究组高于对照组(P<0.01)。治疗后3个月,2组行走乏力、髋关节疼痛、跛行、腰膝酸软积分低于治疗前,且研究组低于对照组(P<0.01)。2组均未出现明显不良反应。结论复方杜仲健骨颗粒联合体外冲击波治疗早期股骨头坏死的临床疗效确切,可更有效地改善患者的临床症状及髋关节功能,且安全性较高。Objective To explore the clinical effect of compound Duzhongjiangu granules combined with extracorporeal shock wave in the treatment of early femoral head necrosis.Methods A total of 84 cases of early femoral head necrosis were divided into control group and study group by random number table method,with 42 cases in each group.The control group received extracorporeal shock wave treatment,and the study group received compound Duzhongjiangu granules on the basis of the control group,both groups were treated for 3 months.Clinical effect,HHS,JOA score,TCM syndrome score before treatment and 3 months after treatment,incidence of adverse reaction were compared between the two groups.Results Total effective rate of study group was 90.48%,which was higher than 71.43%of control group(χ^(2)=4.941,P=0.026).At 3 months after treatment,HHS,JOA score in the two groups were higher than those before treatment,and the study group were higher than those in control group(P<0.01).At 3 months after treatment,scores of walking fatigue,hip pain,limping,soreness of waist and knees in the two groups were lower than those before treatment,and the study group were lower than those in control group(P<0.01).There were no obvious adverse reactions in the two groups.Conclusion Compound Duzhongjiangu granules combined with extracorporeal shock wave have an exact clinical effect in the treatment of early femoral head necrosis,they can more effectively improve clinical symptoms and hip function of patients,and with high safety.
